How can I restore the appearance of sunken calves?
Hello, two months ago, I accidentally fell down four or five steps while going downstairs and hit the front of my right calf.
I immediately applied ice, and the swelling subsided quickly within two to three days.
However, the previously swollen area now has a linear indentation, as the impact caused a slight depression and left a darker mark.
(There was no fracture or sprain at the time.) With summer approaching, it's quite noticeable when I wear skirts, and it doesn't look good.
I would like to ask the doctor if there are any ways to improve this situation.
I'm not sure which department to consult, so I hope you can forgive me if I ask in the wrong place.
Thank you.

Dr. Du Zhiqiang reply Dermatology
Dear Meimei: After a contusion, if there is a depression in the calf, you may observe it for a short period.
However, if it does not improve with warm compresses, it may indicate severe damage to the deeper subcutaneous tissues, resulting in a loss of subcutaneous connective tissue and atrophy.
You may consider consulting a plastic surgeon for options such as injectable fillers, tissue support, or fat grafting, which may help.
Wishing you safety and health.
Sincerely, Dr.

Restoring the appearance of a dented calf after an injury can be a concern for many individuals, especially when it affects self-esteem and body image. Based on your description, it seems that you experienced a soft tissue injury that resulted in a dent and discoloration in your calf. Here are some insights and recommendations from a dermatological perspective on how to manage and potentially improve the appearance of your calf.
Understanding the Injury
When you sustained the injury from falling down the stairs, the impact likely caused damage to the soft tissues, including muscles, fat, and possibly the dermis (the layer of skin beneath the outer layer). The initial swelling you experienced was a natural response to the injury, as the body sends fluids to the area to promote healing. However, once the swelling subsided, the dent you observed may be due to a combination of tissue loss, scarring, and changes in the skin's texture.
Treatment Options
1. Topical Treatments:
- Silicone Gel Sheets or Silicone Gel: These products are often recommended for scar management. They can help flatten and soften scars over time. Apply them once the skin has fully healed, and follow the instructions for use.
- Moisturizers: Keeping the skin hydrated can improve its elasticity and overall appearance. Look for products containing ingredients like hyaluronic acid or glycerin.
2. Massage Therapy:
- Gentle massage of the area can help break down any fibrous tissue that may have formed during the healing process. This can also improve blood circulation, which may aid in the healing of the skin and underlying tissues. However, be cautious not to apply too much pressure, especially if the area is still sensitive.
3. Physical Therapy:
- If the dent is associated with muscle tightness or weakness, consulting a physical therapist may be beneficial. They can provide exercises to strengthen the calf muscles and improve overall function.
4. Dermatological Procedures:
- If the dent remains prominent and you are looking for more immediate results, consider consulting a dermatologist or a plastic surgeon. They may suggest treatments such as:
- Dermal Fillers: These can be injected into the area to restore volume temporarily.
- Microneedling: This procedure can stimulate collagen production and improve skin texture.
- Laser Therapy: Certain laser treatments can help with discoloration and improve the overall appearance of the skin.
5. Sun Protection:
- Protecting the area from sun exposure is crucial, especially if there is discoloration. Use a broad-spectrum sunscreen with an SPF of at least 30 to prevent further darkening of the area.
Monitoring Progress
It's important to monitor the area for any changes. If you notice increased redness, swelling, or pain, it may indicate an underlying issue that requires medical attention. Additionally, if the dent does not improve over time or if you have concerns about the appearance, seeking a consultation with a dermatologist or a plastic surgeon can provide you with tailored options based on your specific situation.
Conclusion
While it may take time for the appearance of your calf to improve, there are several strategies you can employ to aid in the healing process and enhance the cosmetic outcome. Remember that patience is key, as tissue remodeling can take several months. By following a comprehensive approach that includes topical treatments, massage, and possibly professional interventions, you can work towards restoring the appearance of your calf. If you have any further questions or concerns, don't hesitate to reach out to a healthcare professional for personalized advice.
