Urine odor before menstruation?
In recent years, my menstrual cycle has been regular, varying by at most ±3 days (29-32 days), and I have experienced minimal menstrual pain, mostly just some discomfort and lower back pain.
However, at the end of March this year, I suddenly experienced severe menstrual cramps, so intense that pain relievers were ineffective, and the pain began even before the bleeding started.
At that time, I was abroad, and by mid-April, when I returned to Taiwan, my period had already ended.
I still went for a gynecological check-up, and the doctor indicated that my ovaries, uterus, and fallopian tubes were all normal with no abnormalities (I underwent blood tests, ultrasound, and a pelvic exam).
However, both my period in April and this month's cycle have been accompanied by severe menstrual pain! Additionally, I noticed a strong odor in my urine (which I can't quite describe, but it resembles the hormonal scent after sexual intercourse).
This strong urine odor disappears after my period.
I am unsure of the cause of these symptoms.
Furthermore, I had a Pap smear at that time, and the report indicated signs of inflammation, requiring a follow-up examination.
Could this be related? (I currently have a stable sexual partner.)

Dr. Dong Yuhong reply Obstetrics and Gynecology
Hello: There are many causes of dysmenorrhea.
After examination, the doctor indicated that the ovaries, uterus, and fallopian tubes are all normal and show no abnormalities (blood tests, ultrasound, and pelvic exams have all been performed), which means that the test results have not shown any significant abnormalities.
However, if you experience menstrual pain and unusual odors, especially if these symptoms persist or worsen, it is necessary to seek medical attention and treatment.
It is important to understand the causes of certain symptoms, which may take some time.
Inform your primary care physician about your concerns, as this will likely help improve your situation.
Thank you.

Understanding unusual urine odor before menstruation can be a complex issue, as it may involve various physiological and pathological factors. Based on your description, it seems that you have been experiencing significant menstrual pain and a distinct change in the odor of your urine, particularly before your menstrual cycle. Let's break down the potential causes and concerns associated with these symptoms.
Potential Causes of Unusual Urine Odor
1. Hormonal Changes: The menstrual cycle is regulated by hormones, including estrogen and progesterone. These hormones can influence the composition of urine, potentially leading to changes in odor. The strong smell you describe, reminiscent of a hormonal scent, could be related to fluctuations in these hormones as your body prepares for menstruation.
2. Dehydration: If you are not drinking enough fluids, your urine may become concentrated, leading to a stronger odor. This is particularly relevant if you have experienced increased pain and stress, which might affect your hydration levels.
3. Dietary Factors: Certain foods, such as asparagus, garlic, or spices, can alter the smell of urine. If you have changed your diet recently, this could be a contributing factor.
4. Infections: Urinary tract infections (UTIs) can cause a strong, unpleasant odor in urine. Although you mentioned that your gynecological examinations showed no abnormalities, it might be worth considering a UTI, especially if you experience any additional symptoms such as burning during urination, increased frequency, or urgency.
5. Menstrual Cycle Phases: The presence of menstrual blood can also affect the odor of urine. Blood can introduce bacteria and other substances that may change the smell of urine, particularly if there is any residual blood in the urinary tract.
6. Cervical Inflammation: You mentioned that your Pap smear indicated inflammation. Cervical inflammation can sometimes lead to changes in vaginal discharge, which may mix with urine and alter its odor. This could be related to an infection or other inflammatory processes.
Concerns and Recommendations
Given that you have experienced significant menstrual pain and changes in urine odor, it is essential to monitor your symptoms closely. Here are some recommendations:
- Follow-Up with Your Gynecologist: Since your Pap smear indicated inflammation, it would be prudent to follow up with your gynecologist. They may want to conduct further tests to rule out any infections or other underlying conditions that could be contributing to your symptoms.
- Hydration: Ensure you are adequately hydrated, especially leading up to your menstrual cycle. This can help dilute your urine and may reduce any strong odors.
- Dietary Review: Consider any recent changes in your diet that might be contributing to the odor. Keeping a food diary could help identify any correlations.
- Monitor Symptoms: Keep track of your menstrual cycle, pain levels, and any changes in urine odor or other urinary symptoms. This information can be valuable for your healthcare provider in diagnosing any potential issues.
- Consider a Urologist Consultation: If the unusual urine odor persists or if you develop additional urinary symptoms, consulting a urologist may be beneficial to rule out any urinary tract issues.
In summary, while hormonal changes related to your menstrual cycle are likely contributing factors to the unusual urine odor, it is essential to consider other potential causes, including infections and dietary influences. Regular follow-up with your healthcare provider will help ensure that any underlying issues are addressed appropriately.
