Ankle scar
Hello Doctor: About a year ago, I injured my left ankle from a fall, approximately 1.5cm x 1.5cm in size.
Although the skin was intact, it seemed like there was internal bleeding.
At the time, I didn't pay much attention to it (thinking that since the skin wasn't broken, it was fine).
After about half an hour, I realized that walking was very painful (similar to typical trauma pain).
I applied some ice, but didn't seek further treatment.
Since there was no visible wound, I didn't purchase any silicone gel sheets.
After the internal bleeding resolved and the wound healed, I noticed it had developed into a raised scar.
I bought over-the-counter scar sheets (Ningban Ning) and used them for about a month (the instructions indicated they should only be applied after the wound has healed), but perhaps due to the constant movement of my ankle, I felt that the scar sheets had no effect at all.
After another six months, I purchased scar gel to apply, and gradually the scar became flatter and the color faded somewhat, but it is still very noticeable when I wear a short skirt.
If I want the scar to be flatter and the color to be lighter, are there any medications I can apply or injections I can receive? (I understand that it is impossible to have no scar at all, but is there a way to make it blend in with my skin tone?) If I visit a dermatology clinic, can the doctor help me with this? Or are there any medications I can buy to apply myself? Thank you.

Dr. Huang Ruiyun reply Dermatology
Hello: You can visit a dermatologist or a plastic surgeon to let the doctor understand the actual situation.
In fact, this type of scar may still improve naturally over a period of two years without any special treatment.
Wishing you well.

More Info
When it comes to treating ankle scars, especially those resulting from injuries like falls, there are several effective options available. Scarring can be a frustrating consequence of injuries, and while complete removal of scars is often not possible, there are methods to significantly improve their appearance.
Understanding Scarring
Scars form as part of the body’s natural healing process. When the skin is injured, the body produces collagen to repair the damaged tissue. In some cases, this collagen can form in excess, leading to raised or discolored scars. The type of scar you described, which is raised and noticeable, is likely a hypertrophic scar or keloid, depending on its characteristics.
Treatment Options
1. Topical Treatments:
- Silicone Gel Sheets or Silicone Gel: These are often recommended for scar management. They work by hydrating the scar and creating an optimal healing environment. You mentioned using a silicone gel sheet, which is a good start. Consistent use over several months can lead to improvements.
- Scar Creams: Products containing ingredients like onion extract (found in products like Mederma) or vitamin E may help in reducing the appearance of scars, although results can vary from person to person.
2. Injections:
- Steroid Injections: If the scar is raised, corticosteroid injections can help flatten it. This treatment is usually administered by a dermatologist and may require multiple sessions.
- Hyaluronic Acid Fillers: In some cases, fillers can be used to improve the appearance of scars, especially if they are indented.
3. Laser Treatments:
- Laser Therapy: This is a more advanced option that can help reduce redness and flatten scars. Different types of lasers are used depending on the scar type and skin type. This treatment typically requires several sessions and should be performed by a qualified dermatologist.
4. Microneedling:
- This procedure involves using fine needles to create micro-injuries in the skin, which stimulates collagen production and can improve the texture and appearance of scars over time.
5. Chemical Peels:
- Chemical peels can help improve the surface of the skin and reduce discoloration associated with scars. This treatment should also be performed by a qualified professional.
Consultation with a Dermatologist
Given that your scar has persisted and you are looking for more effective treatments, consulting a dermatologist would be a wise step. They can assess the scar's characteristics and recommend the most appropriate treatment options tailored to your needs. They may also provide treatments that are not available over the counter.
Home Care and Lifestyle Adjustments
While waiting for your appointment or if you prefer to start at home, consider the following:
- Sun Protection: Protecting the scar from sun exposure is crucial. UV rays can darken scars, making them more noticeable. Use a broad-spectrum sunscreen with at least SPF 30 on the scar when exposed to sunlight.
- Moisturization: Keeping the scar moisturized can help improve its appearance. Use a fragrance-free moisturizer or a silicone-based product.
- Patience: Scars can take time to mature and fade. It may take several months to see significant changes, so consistency is key.
Conclusion
While it may not be possible to completely eliminate the scar, various treatments can help improve its appearance. From topical treatments to professional procedures, there are multiple avenues to explore. Consulting with a dermatologist will provide you with the best options tailored to your specific situation. Remember, the goal is to make the scar less noticeable and more aligned with your natural skin tone.
