Nasal discomfort
Hello Doctor: I have been experiencing discomfort in my nose for about two years, primarily with persistent nasal congestion on the left side.
Additionally, I feel a sensation of pressure behind my left eye, as if something is pushing against it, which has been occurring for about the same duration as my nasal discomfort.
When I close my eyes and apply pressure to my eyeball with my palm, I can sense what seems to be fluid being compressed behind the eyeball.
Furthermore, when I open my mouth wide or clench my teeth, I feel that this suspected fluid is being compressed again, accompanied by a sound of fluid being pressed.
A year ago, I underwent a nasal endoscopy, which revealed polyps, but the pathology report showed no abnormalities.
I also had tests for Epstein-Barr virus antibodies that came back normal, and a CT scan showed no abnormalities.
I would like to know if there could be other factors causing this, especially the sensation of fluid pressure behind the eyeball.
What methods of examination could be performed, or do you have any other suggestions? Thank you, Doctor.

More Info
Nasal discomfort, particularly when accompanied by symptoms such as nasal congestion and pressure behind the eyes, can be quite distressing. Based on your description, it seems you have been experiencing these symptoms for about two years, with a notable feeling of pressure behind your left eye and a sensation of liquid being compressed when you perform certain movements. Given your history of nasal polyps and the various tests you've undergone, including nasal endoscopy and CT scans, it's essential to consider several potential causes and solutions.
Possible Causes of Your Symptoms
1. Chronic Sinusitis: Even if your CT scan did not show significant abnormalities, chronic sinusitis can still occur without clear imaging findings. This condition can lead to persistent nasal congestion and pressure, which may explain the discomfort behind your eye.
2. Nasal Polyps: Although your nasal polyps were found to be benign, they can still contribute to nasal obstruction and sinus drainage issues. If they are large or located in a way that obstructs normal airflow, they can exacerbate your symptoms.
3. Eustachian Tube Dysfunction: This condition can cause a feeling of fullness or pressure in the ears and may also contribute to sensations of pressure behind the eyes. It occurs when the tube that connects the middle ear to the back of the nose becomes blocked or does not function properly.
4. Allergic Rhinitis: Allergies can lead to inflammation of the nasal passages, causing congestion and pressure. If you have not been evaluated for allergies, this could be a significant factor in your symptoms.
5. Other Factors: Conditions such as migraines or tension headaches can also cause referred pain to the nasal area and behind the eyes. Additionally, stress and anxiety can exacerbate these symptoms.
Recommended Solutions and Next Steps
1. Consult an ENT Specialist: Since you have a history of nasal issues, a follow-up with an ear, nose, and throat (ENT) specialist is crucial. They may recommend further imaging studies or a repeat nasal endoscopy to assess any changes since your last evaluation.
2. Nasal Corticosteroids: If you have not already been prescribed nasal corticosteroids, these can help reduce inflammation in the nasal passages and sinuses, potentially alleviating your symptoms.
3. Saline Nasal Irrigation: Regularly using saline nasal sprays or performing nasal irrigation can help keep the nasal passages moist and clear out mucus, which may relieve some of the pressure and discomfort.
4. Allergy Testing: If allergies are suspected, consider undergoing allergy testing. Identifying and avoiding allergens can significantly improve symptoms.
5. Eustachian Tube Dysfunction Management: If Eustachian tube dysfunction is suspected, techniques such as the Valsalva maneuver (gently blowing while pinching the nose) can help equalize pressure. In some cases, a referral to an audiologist or a specialist in ear disorders may be warranted.
6. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort. However, consult your doctor before starting any new medication.
7. Lifestyle Modifications: Staying hydrated, using a humidifier, and avoiding known irritants (like smoke or strong odors) can also help reduce nasal discomfort.
Conclusion
Your symptoms of nasal discomfort and pressure behind the eye can stem from various underlying issues, including chronic sinusitis, nasal polyps, or Eustachian tube dysfunction. A thorough evaluation by an ENT specialist is essential to determine the exact cause and appropriate treatment. In the meantime, consider implementing some of the recommended solutions to help alleviate your symptoms. Remember, addressing both the physical and potential psychological aspects of your discomfort is crucial for comprehensive care.
