Phimosis with suspected suppuration?

Hello Doctor: I just finished taking a shower not long ago and suddenly noticed that my penis is experiencing sharp pain.
Upon inspection, I found that the tip of the foreskin is swollen.
It hurts intensely when urinating or when it comes into contact with water, and there is also slight pain when at rest.
However, there is no significant pain when I apply pressure to it.
The swollen area is approximately 0.6 cm in length and 0.8 cm in width.
It appears white and slightly translucent.
I suspect it might be an abscess, but it feels soft and does not burst when pressed.
What should I do to resolve this issue?

It may be a case of balanitis, but it is still necessary to visit a urology clinic for evaluation and treatment by a physician.

Swollen foreskin, or preputial swelling, can be a concerning issue, especially when accompanied by pain and discomfort. Based on your description, it seems you are experiencing localized swelling at the tip of the foreskin, which is causing significant pain during urination and when in contact with water. Here’s a comprehensive understanding of the potential causes and treatment options for your condition.

Causes of Swollen Foreskin
1. Infection: One of the most common causes of swelling in the foreskin is an infection, which can be bacterial or fungal. Balanitis, an inflammation of the glans (the head of the penis), can lead to swelling of the foreskin. Symptoms often include redness, swelling, and pain.

2. Allergic Reaction: Sometimes, irritation from soaps, lotions, or even certain fabrics can cause an allergic reaction, leading to swelling and discomfort.

3. Phimosis: This condition occurs when the foreskin cannot be easily retracted over the glans. It can lead to swelling and pain, especially if there is an infection or inflammation.

4. Trauma or Injury: Any recent trauma to the area, even minor, can result in swelling. This could be from vigorous sexual activity or even aggressive cleaning.

5. Cysts or Abscesses: The presence of a cyst or an abscess can also cause localized swelling. An abscess is a collection of pus that can form due to infection and may feel soft and tender.

Treatment Options
1. Maintain Hygiene: Keeping the area clean is crucial. Gently wash the area with mild soap and water. Avoid harsh soaps or products that may irritate the skin further.

2. Topical Treatments: Over-the-counter antifungal or antibacterial creams may help if an infection is suspected. However, it’s essential to consult a healthcare provider before applying any medication.

3. Cold Compress: Applying a cold compress to the swollen area can help reduce swelling and alleviate pain. Make sure to wrap ice in a cloth to avoid direct contact with the skin.

4. Pain Relief: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and discomfort.

5. Avoid Irritants: If you suspect that soaps or other products are causing irritation, discontinue their use until the swelling subsides.

6. Consult a Healthcare Provider: Given the symptoms you described, especially the severe pain during urination, it is advisable to seek medical attention. A healthcare provider can perform a physical examination and may recommend further tests, such as a swab or urine test, to determine the underlying cause.

7. Possible Circumcision: In cases of recurrent infections or phimosis, circumcision may be recommended as a long-term solution to prevent future issues.

When to Seek Immediate Care
If you experience any of the following symptoms, seek medical attention promptly:
- Severe pain that does not improve with over-the-counter medications.

- Fever or chills, which may indicate a systemic infection.

- Difficulty urinating or blood in the urine.

- Rapidly increasing swelling or redness.

In conclusion, while swollen foreskin can be caused by various factors, maintaining hygiene and seeking medical advice are crucial steps in addressing the issue. Early intervention can prevent complications and ensure a swift recovery.
