Frenulum breve rupture?

Hello, doctor.
Today while showering, I was washing my glans and possibly due to improper force and direction, I suddenly felt severe pain and then experienced continuous bleeding.
I realized that the entire frenulum was almost completely torn apart.
The separation point is roughly at the junction of the glans and the foreskin.
If I pull it apart, I can see that there is about a one-centimeter gap between the two torn frenulum segments.
It has been over an hour, and I have been bleeding continuously; however, the bleeding has gradually lessened under my constant pressure.
Doctor, would applying ice to the affected area help stop the bleeding? Do I need any ointment? Generally, how long does it take for this to heal on its own? Thank you!

Frenulum tears, particularly in the genital area, can be quite painful and concerning. The frenulum, which is the band of tissue connecting the underside of the penis to the foreskin, can tear due to excessive force or improper handling during washing or sexual activity. In your case, it seems that the tear occurred during a vigorous washing session, leading to significant pain and bleeding.

Causes of Frenulum Tears
Frenulum tears can occur for several reasons:
1. Excessive Force: As you experienced, vigorous washing or pulling can lead to tears.

2. Sexual Activity: Sometimes, during sexual intercourse, the frenulum can be overstretched or torn.

3. Infections or Inflammation: Conditions that cause swelling or irritation can make the frenulum more susceptible to tearing.

Immediate Treatment
1. Control Bleeding: It’s good to hear that you have been applying pressure to the area, which is the first step in controlling bleeding. If the bleeding continues or worsens, it is crucial to seek medical attention.

2. Ice Application: Applying ice to the area can help reduce swelling and numb the pain, which may indirectly help with bleeding by constricting blood vessels. However, be cautious not to apply ice directly to the skin; wrap it in a cloth or use a cold pack.

3. Avoid Further Irritation: Refrain from any activities that may further irritate the area, including vigorous washing or sexual activity, until healing has occurred.

Medications and Topical Treatments
You may not necessarily need a specific ointment unless there is a risk of infection. However, if you notice any signs of infection (increased redness, swelling, pus, or fever), you should consult a healthcare provider. Over-the-counter pain relievers like ibuprofen or acetaminophen can help manage pain and inflammation.

Healing Time
The healing time for a frenulum tear can vary depending on the severity of the tear. Generally, minor tears may heal within a few days to a week, while more significant tears might take longer. It’s essential to keep the area clean and dry to promote healing. If you notice that the bleeding does not stop, or if the pain worsens, it is advisable to seek medical attention.

When to Seek Medical Help
- Persistent Bleeding: If the bleeding does not stop after applying pressure for 10-15 minutes.

- Signs of Infection: If you notice increased redness, swelling, or discharge.

- Severe Pain: If the pain becomes unbearable or does not improve with over-the-counter pain relief.

Conclusion
In summary, while minor frenulum tears can often heal on their own with proper care, it is crucial to monitor your symptoms closely. Applying ice can help with pain and swelling, and controlling bleeding through pressure is essential. If you have any concerns about your symptoms or if they worsen, do not hesitate to seek medical advice. Your health and comfort are paramount, and a healthcare professional can provide the best guidance tailored to your situation.
