Do I need to undergo treatment for the upper teeth before getting dentures for the lower teeth?
Doctor, my lower left first and second molars have been missing for about two years.
Today, I went to see a dentist who said that because the teeth have been missing for so long, the upper teeth have grown downwards.
This has caused the lower teeth to take on a somewhat concave shape.
Therefore, in order to treat the lower teeth, the upper teeth must be addressed first (I also had the last upper wisdom tooth extracted today).
The dentist suggested that the two upper teeth be shortened before making crowns, so that I can then get dentures for the lower teeth.
Is this approach by my dentist the best option for me? I also asked if it would be possible to proceed directly with the lower teeth without addressing the upper ones, and he advised against it.
My friends are very concerned that I might be getting scammed by the dentist into doing two extra teeth.
Thank you for your response, Doctor.

Hello, Miss: The lower molars have experienced a prolonged absence of dental buds, causing the upper teeth to grow downwards towards the molars.
If the molars grow too tall, there will be insufficient space to place dentures on the lower molars.
Forcing the placement of dentures on the lower molars may result in uneven occlusal heights, which could lead to occlusal interference over time.
Therefore, it is recommended to shorten the two upper molars and place crowns on them before proceeding with the dentures for the lower molars.
Wishing you peace and happiness!

When considering dental treatment, especially in the context of getting lower dentures, it is essential to understand the relationship between your upper and lower teeth. Your dentist's recommendation to address the upper teeth before proceeding with lower dentures is based on sound dental principles.
In your case, the upper teeth have over-erupted due to the absence of the lower first and second molars for an extended period. This over-eruption can lead to several complications, including misalignment of the bite, difficulty in chewing, and potential discomfort. When the upper teeth extend downwards, they can create an uneven occlusion (bite), which can complicate the fitting and function of any lower dentures you may receive.
Your dentist's approach to first shorten the upper teeth before making lower dentures is a common and prudent strategy. By adjusting the upper teeth, your dentist aims to create a more balanced bite, which is crucial for the proper function of the dentures. If the upper teeth are not adjusted, the lower dentures may not fit correctly, leading to discomfort, difficulty in chewing, and potential issues with speech.
It is also important to consider that if you were to proceed with lower dentures without addressing the upper teeth, you might face complications later on. The dentures may not stay in place properly, and you could experience increased wear on the dentures due to the misalignment. Furthermore, the pressure from the dentures could exacerbate any existing issues with the upper teeth, leading to further dental problems.
Regarding your concern about whether your dentist is recommending unnecessary treatments, it is always a good practice to seek a second opinion if you feel uncertain. However, the rationale provided by your dentist aligns with standard dental practices. Dentists often recommend treating the entire dental arch (both upper and lower) to ensure that any prosthetic devices, such as dentures, function effectively and comfortably.
In summary, your dentist's recommendation to treat the upper teeth before proceeding with lower dentures is a sound approach aimed at ensuring the best possible outcome for your dental health. Proper alignment and occlusion are critical for the success of dentures, and addressing the upper teeth first can help prevent complications down the line. If you still have concerns, consider discussing them further with your dentist or seeking a second opinion from another dental professional. Your oral health is paramount, and it is essential to feel confident in the treatment plan you choose.
