Premature ejaculation issues?
Hello Doctor, I have been experiencing issues with pre-ejaculation over the past two months.
While sitting at my desk during the day, I suddenly notice the smell of semen.
There have been a few instances where I specifically went to the restroom to check, and I found a small amount of semen leaking out, possibly about a drop.
This occurs without any erection.
Initially, I didn't think much of it since I didn't feel anything, but now it seems to happen every day.
While I'm working, I can smell it, and I'm worried that my colleagues might notice when they come to talk to me.
Is it normal to have leakage without an erection? Could there be an issue with my body? Thank you!

Maguire: Hello, may I ask if it has been a long time since you last ejaculated? Generally, for normal males, after a period of semen production without ejaculation, there can indeed be occurrences of seminal leakage.
When there is excessive production, leakage can happen at any time.
Wearing loose-fitting clothing can help reduce local stimulation and decrease erections, which should improve the situation.
If there are any unclear points, feel free to ask again, and we can further discuss and research this together.

Pre-ejaculation, or the release of pre-seminal fluid, is a common phenomenon that many men experience. This fluid is produced by the Cowper's glands and serves several purposes, including lubrication and neutralizing acidity in the urethra. However, the situation you describe—experiencing leakage of this fluid without any sexual arousal or erection—can be concerning and may warrant further exploration.
Firstly, it is important to understand that the involuntary release of pre-ejaculatory fluid can occur for various reasons. In some cases, it may be linked to heightened sensitivity or arousal, even if one does not consciously feel aroused. Stress, anxiety, or even certain physical positions can trigger this response. Given that you mentioned this has been happening frequently over the past two months, it could be related to psychological factors, such as increased anxiety about your sexual health or performance.
Additionally, hormonal changes or fluctuations in testosterone levels can also impact sexual function and the production of seminal fluid. If your body is producing more pre-ejaculatory fluid than usual, it may be a sign of hormonal changes or an increased sensitivity of the glands responsible for its production.
Another possibility is that you may be experiencing a condition known as "seminal leakage," which can occur due to various factors, including pelvic floor dysfunction or issues with the prostate. In some cases, men may experience this leakage without any accompanying erection or sexual arousal, which can be perplexing and concerning.
It is also worth noting that certain lifestyle factors can contribute to this issue. For example, a sedentary lifestyle, poor diet, and lack of exercise can negatively impact overall sexual health. Engaging in regular physical activity, maintaining a balanced diet, and managing stress levels can all contribute to improved sexual function and may help alleviate some of the concerns you are experiencing.
If you are noticing a consistent pattern of leakage, especially if it is accompanied by any discomfort or changes in your sexual function, it is advisable to consult a urologist or healthcare professional. They can perform a thorough evaluation, which may include a physical examination, hormone level testing, and possibly imaging studies to rule out any underlying conditions.
In summary, while the involuntary release of pre-ejaculatory fluid can be normal for some men, the frequency and context in which it is occurring in your case suggest that it may be beneficial to seek medical advice. A healthcare professional can help determine if there is an underlying issue that needs to be addressed and provide guidance on lifestyle changes or treatments that may help improve your situation. Remember, open communication with your healthcare provider is key to understanding and managing any concerns related to sexual health.
