White discoloration above the anus?
Hello, doctor.
For the past two days, I've been experiencing pain above the anus (when wiping).
I took a look with a camera and noticed some white areas, but I couldn't see clearly what they were.
When I wiped with a wet tissue, there was a bit of an odor (similar to the smell when eczema gets infected).
After wiping harder, I noticed some bleeding.
I'm not sure what this condition could be.

Hello: One of the agreements that consulting physicians on this website must follow is that consultations are limited to medical inquiries only, and do not include diagnoses or prescriptions.
If a consultation is needed, patients are advised to visit a hospital.
Therefore, for a diagnosis, it is recommended to see a colorectal surgeon in person.
Wishing you the best.

More Info
When experiencing pain and discoloration above the anus, it is essential to consider several potential causes, as this area can be affected by various conditions. Based on your description, it sounds like you may be dealing with a few possibilities, including hemorrhoids, anal fissures, or skin infections.
1. Hemorrhoids: These are swollen veins in the lower rectum or anus that can cause discomfort, itching, and bleeding, especially during bowel movements. The presence of pain and blood when wiping could indicate that you have external hemorrhoids, which are located outside the anal canal. They can become irritated and painful, especially if there is excessive wiping or straining during bowel movements.
2. Anal Fissures: An anal fissure is a small tear in the lining of the anus, often caused by passing hard stools or straining during bowel movements. Symptoms typically include sharp pain during and after bowel movements, as well as bleeding. The white discoloration you observed could be a result of irritation or inflammation around the fissure.
3. Skin Infections or Conditions: Conditions such as fungal infections, dermatitis, or even sexually transmitted infections can cause irritation and discoloration in the anal area. The "white" appearance you noted could be due to a fungal infection, which often presents with itching and a distinct odor.
4. Abscess or Fistula: An anal abscess is a painful condition that occurs when a pocket of pus forms near the anus, often due to an infection. Symptoms include severe pain, swelling, and sometimes fever. If an abscess does not drain properly, it can lead to the formation of a fistula, which is an abnormal connection between the anal canal and the skin.
Given the symptoms you described, including pain, bleeding, and a noticeable odor, it is crucial to seek medical attention. A healthcare provider can perform a thorough examination, which may include a visual inspection and possibly a digital rectal exam, to determine the exact cause of your symptoms. They may also recommend diagnostic tests, such as a stool test or imaging studies, if necessary.
In the meantime, here are some general recommendations to help alleviate discomfort:
- Avoid Straining: Ensure that you are not straining during bowel movements. A high-fiber diet and adequate hydration can help soften stools and reduce the risk of constipation.
- Gentle Cleaning: Use gentle, unscented wipes or warm water to clean the area after bowel movements. Avoid harsh soaps or excessive scrubbing.
- Warm Sitz Baths: Taking warm sitz baths can help soothe the area and promote healing.
- Over-the-Counter Treatments: You may consider using over-the-counter creams or ointments designed for hemorrhoids or fissures, which can provide temporary relief.
It is essential to avoid self-diagnosing and treating without professional guidance, as some conditions may require specific treatments or interventions. If your symptoms persist or worsen, or if you experience significant bleeding, fever, or severe pain, please seek immediate medical attention.
