Anal and hemorrhoidal issues?

Hello Doctor: I developed hemorrhoids during my first pregnancy.
After giving birth, the external hemorrhoids became enlarged, so I underwent surgery for removal.
Later, during my second pregnancy, I experienced hemorrhoids again, this time internal.
I had rubber band ligation done three times.
However, I still occasionally experience itching around the anus.
Applying ointment has improved the symptoms, but there is a very foul odor coming from the anal area.
I often notice it when urinating, especially after wiping with toilet paper, confirming that the smell is emanating from the anus.

Interestingly, my underwear used to have dark stains, and initially, I thought it was an issue related to my gynecological health.
Later, I realized that the stains were coming from the anal area.
The dark stains on my underwear are difficult to wash out.
I feel puzzled and went to a large hospital to see a colorectal specialist, but the doctor said it wasn't as serious as I described.
I visited two hospitals, and neither provided an explanation for the dark stains on my underwear.
Therefore, I am still very confused and would like to ask what the dark stains on my underwear are, whether they are related to hemorrhoids, and if the strong odor is connected in any way.

Thank you for your response.
Based on your description, it is not possible to determine whether the issue is related to the original hemorrhoids or if there are other problems.
I recommend that you visit the outpatient surgery department of the hospital for further examination.

More Info

It sounds like you're experiencing a complex set of symptoms related to your history of hemorrhoids and recent childbirth. Let's break down your concerns regarding hemorrhoids, odor, discoloration, and their potential connections.

Understanding Hemorrhoids
Hemorrhoids are swollen veins in the lower rectum and anus, which can be either internal or external. Internal hemorrhoids are usually painless but can cause bleeding, while external hemorrhoids can be painful and may protrude outside the anal opening. Given your history of both external and internal hemorrhoids, it’s understandable that you are experiencing discomfort and other symptoms.

Odor and Discoloration
The foul odor you are noticing could be attributed to several factors:
1. Hygiene: After bowel movements, if the area is not cleaned thoroughly, residual stool can lead to odor. It’s essential to maintain good hygiene, especially after using the restroom. Gentle cleaning with water or using moist wipes can help.

2. Infection: Sometimes, if there is a fissure or if the hemorrhoids are inflamed, it can lead to an infection, which may produce a foul smell. If you notice any unusual discharge or persistent odor, it’s crucial to consult a healthcare provider.

3. Skin Irritation: The skin around the anus can become irritated due to moisture, friction, or the presence of hemorrhoids. This irritation can sometimes lead to a foul odor.

4. Diet: Certain foods can affect body odor. Spicy foods, garlic, and onions can contribute to a stronger smell.

The Black Discoloration on Underwear
The black discoloration you are observing on your underwear could be due to several reasons:
1. Blood: If you have been experiencing bleeding from your hemorrhoids, the blood may dry and appear dark. This could explain the discoloration on your underwear.

2. Fecal Matter: If there is any leakage of stool due to the hemorrhoids, it can lead to staining. This is particularly common if there is any rectal prolapse or if the hemorrhoids are causing difficulty in complete evacuation.

3. Skin Condition: Sometimes, skin conditions or infections can lead to discoloration. If there is any fungal infection or other dermatological issue, it could also contribute to the staining.

Recommendations
1. Consult a Specialist: Since you have already seen a colorectal specialist, it may be beneficial to seek a second opinion or consult a gastroenterologist who specializes in gastrointestinal issues. They can perform a thorough examination to rule out any underlying conditions.

2. Hygiene Practices: Ensure that you are practicing good hygiene. After bowel movements, consider using a bidet or moist wipes to clean the area gently. Avoid harsh soaps that can irritate the skin.

3. Dietary Adjustments: Monitor your diet and see if certain foods exacerbate your symptoms. Staying hydrated and consuming a high-fiber diet can help prevent constipation and reduce strain during bowel movements.

4. Follow-Up Care: If your symptoms persist, including the odor and discoloration, follow up with your healthcare provider. They may recommend further tests, such as a stool test or imaging studies, to investigate the cause of your symptoms.

5. Topical Treatments: Continue using the prescribed ointments or creams for hemorrhoids, as they can help reduce inflammation and discomfort. If you notice any adverse effects or if the symptoms do not improve, consult your doctor for alternative treatments.

In summary, while hemorrhoids can cause discomfort and associated symptoms, the odor and discoloration you are experiencing warrant further investigation. Maintaining good hygiene, monitoring your diet, and seeking appropriate medical advice will be crucial in managing your symptoms effectively.
