Retinal Issues: Symptoms and Treatments for High Myopia - Ophthalmology

Retinal-related issues?

Hello, Dr.
Hu: I have high myopia in both eyes, with a degree of approximately over 1400 diopters.
In early September last year, I experienced symptoms of hemorrhage in the macula of my right eye.
After examination by a doctor, it was determined to be dry hemorrhage, and no related treatment was administered.
Around October, I noticed an increase in floaters, but after a thorough fundus examination, the doctor found that there was traction between the vitreous and the retina in my right eye.
To prevent detachment, I underwent laser surgery.
Since then, I have had regular fundus examinations every three months, and the doctor has reported no signs of detachment.
Here are my questions:
1.
Recently, I have noticed that after prolonged use of the computer or reading/watching TV, I see wavy lines or alternating black and white lines.
Additionally, when looking at a colored cover, such as a red cover, after a long time, I see a red shadow when I look away.
What could be causing this condition?
2.
Recently, when I blink, the light in both eyes appears elongated, sometimes extending to the ground.
What could be causing this condition? Is there any treatment available?
3.
When I close my left eye, I sometimes experience a pulling or flickering sensation in the lower left area of my left eye, similar to something rotating in that area.
What could be causing this condition?
Thank you, doctor, for your response.
Thank you.

Hello Mr.
Hsiao: 1.
Staring at a red cover for a period of time and then seeing a shadow afterward may be due to visual persistence.
2.
I do not fully understand your description; it could be caused by astigmatism induced by tears.
3.
I recommend a further dilated eye examination of your left eye to determine if there is any traction on the vitreous or retina, or other pathological conditions.
Wishing you health and safety.

Hello, thank you for your detailed questions regarding your eye health, particularly in the context of high myopia. Given your history of significant myopia (over 1400 degrees) and the recent complications you've experienced, it's important to address your concerns with a comprehensive understanding of the potential implications and symptoms associated with high myopia.

1. Visual Disturbances with Prolonged Screen Time: The symptoms you're experiencing, such as seeing wavy lines or alternating black and white patterns after prolonged use of computers or reading, could be attributed to several factors. One possibility is visual fatigue or digital eye strain, which is common among individuals who spend extended periods focusing on screens. This can lead to temporary distortions in vision, including the perception of wavy lines or shadows. Additionally, the presence of floaters (which you mentioned has increased) can also contribute to these visual disturbances, especially in high myopia, where the vitreous gel may be more prone to changes and traction on the retina. It's advisable to take regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for at least 20 seconds.

2. Light Distortion and Visual Effects: The phenomenon of light appearing elongated or distorted when you blink could be related to several factors, including changes in the shape of your cornea or lens due to the high degree of myopia. This distortion can also be exacerbated by fatigue or dryness in the eyes, which is common in individuals who spend a lot of time in front of screens. Ensuring that you maintain proper hydration and using artificial tears can help alleviate some of these symptoms. If the distortion persists or worsens, further evaluation by your eye care provider may be necessary to rule out any underlying issues.

3. Flashes and Floaters: The sensation of seeing flashes or a swirling motion in your peripheral vision, especially when closing one eye, is concerning and should be monitored closely. This could indicate vitreous traction on the retina, which is not uncommon in high myopia. While you have undergone laser treatment to prevent retinal detachment, it is crucial to continue regular follow-ups with your ophthalmologist. Any new or worsening symptoms, such as an increase in the number of flashes or floaters, should prompt immediate evaluation, as they can be indicative of retinal issues that may require intervention.

In summary, while many of the symptoms you are experiencing can be attributed to the effects of high myopia and its associated complications, it is essential to maintain regular check-ups with your eye care specialist. They can provide tailored advice and treatment options based on the ongoing assessment of your eye health. Additionally, practicing good eye hygiene, taking breaks during prolonged visual tasks, and ensuring proper lighting can help mitigate some of the discomfort and visual disturbances you are experiencing. If you notice any significant changes in your vision or new symptoms, do not hesitate to seek immediate medical attention. Your proactive approach to monitoring your eye health is commendable, and I wish you the best in managing your condition.
