Urological Concerns: Common Questions About Sexual Health - Urology

Hello, Dr.
Hsu.
I have a few questions:
1.
The frenulum connecting the glans to the foreskin appears to have a bruised black color, extending to the scrotum, and there is a continuous area along the underside of the penis that looks bruised but is not painful.
What could this phenomenon be?
2.
I masturbate on average once every 1 to 2 days, usually using my hand to apply pressure and rub the penis.
Could this be the cause of the aforementioned phenomenon?
3.
After each masturbation session, I find myself coughing the next day, feeling as if there is phlegm in my lungs.
If I masturbate twice in one day, the coughing is even more pronounced and frequent.
What could be the reason for this?
4.
I usually ejaculate quickly during masturbation.
Will I also ejaculate quickly during actual intercourse?
5.
I have heard of a technique where one pauses just before ejaculation during masturbation, rests for a while, and then continues to help prolong endurance.
Is this method effective? Are there any side effects? Are there other self-training methods available?
Thank you for your assistance.

Xiao Y: Hello.
1.
It is difficult to make a definitive assessment.
Is there any history of trauma? How long has this been occurring? 2.
Theoretically, it shouldn't happen unless there is significant trauma or an underlying coagulation disorder or other related diseases.
3.
Ejaculation and coughing??? I'm not clear on that; I will look into it further.
4.
Not necessarily.
Sometimes, actual intercourse may lead to quicker ejaculation; other times, it may take longer.
It depends on both partners' experience and technique.
5.
It should be effective.
However, why is such training necessary? The timing of ejaculation, the firmness of the erection, and the duration of the erection are related to the cooperation, technique, and experience of both partners.
Unilateral training...

If there are any unclear points, feel free to ask again, and we can discuss and research further.

When it comes to urological concerns and sexual health, it’s essential to address each question with care and a thorough understanding of the underlying issues. Here’s a detailed response to your inquiries:
1. Bruising or Dark Discoloration on the Frenulum: The appearance of dark bruising or discoloration on the frenulum (the band of tissue connecting the glans to the foreskin) and extending to the scrotum can be concerning. This could be due to several factors, including trauma from vigorous sexual activity or masturbation, or it could be a sign of a vascular issue. If there is no pain associated with this discoloration, it may not be an immediate concern, but it is advisable to monitor the area for any changes. If the discoloration persists or worsens, or if you experience pain, swelling, or any other symptoms, it is crucial to consult a healthcare professional for a proper evaluation.

2. Masturbation Technique and Its Effects: Engaging in frequent masturbation, especially with a tight grip, can lead to irritation or trauma to the penile tissues, which might contribute to the bruising or discoloration you are experiencing. While masturbation is a normal sexual activity, the technique and frequency can impact your genital health. It’s important to practice gentle techniques and allow for adequate recovery time between sessions to prevent injury.

3. Coughing After Masturbation: Experiencing a cough or a sensation of phlegm in the lungs after masturbation is unusual and could be related to a few factors. It might be a reflex response to the physical exertion associated with sexual activity, or it could be related to anxiety or stress surrounding the act. If this symptom persists, it would be wise to consult a healthcare provider to rule out any underlying respiratory issues or allergies.

4. Premature Ejaculation Concerns: It is common for individuals to experience quicker ejaculation during masturbation compared to sexual intercourse. This can be due to the different levels of arousal, stimulation, and psychological factors involved in each activity. If you find that you are consistently ejaculating quickly during sexual encounters, it may be beneficial to explore techniques to help manage this, such as focusing on relaxation and communication with your partner.

5. Techniques to Extend Ejaculation Duration: The technique of pausing during masturbation when nearing ejaculation (often referred to as the "stop-start" method) can indeed help some individuals learn to control their ejaculation better. This method allows you to build awareness of your body’s signals and can potentially lead to improved control during intercourse. There are also other techniques, such as pelvic floor exercises (Kegel exercises), which can strengthen the muscles involved in ejaculation and improve control. It’s important to approach these methods with patience and practice, as results may vary from person to person.

In conclusion, while many of your concerns are common and can be addressed with lifestyle adjustments and techniques, it is always best to consult with a healthcare professional for personalized advice and to rule out any serious conditions. Maintaining open communication about sexual health and being proactive about any changes in your body is key to ensuring your overall well-being.
