Glans penis

Hello, Dr.
Hsu.
Two days ago, I noticed a few small white spots (about 1mm raised) and some red spots (1mm in size, not raised) near the urethral opening of my glans.
They are not itchy or painful.
I have not engaged in sexual intercourse or received a blood transfusion.
Could you please tell me what this condition might be? I am quite worried.
Thank you.

Understanding small white and red bumps on the glans can be concerning, especially when they appear suddenly. Based on your description, the small white bumps (approximately 1mm in size) and red spots (also about 1mm, but not raised) near the urethral opening of the glans are likely benign and may not indicate a serious condition. However, it is essential to consider several potential causes and factors.

1. Pearly Penile Papules: These are small, harmless, dome-shaped bumps that can appear around the glans and are often mistaken for warts or other infections. They are a normal anatomical variant and are not associated with any disease or infection. They typically do not cause any symptoms such as itching or pain.

2. Fordyce Spots: These are small, painless, pale bumps that can appear on the glans and are caused by enlarged sebaceous (oil) glands. They are completely harmless and do not require treatment.

3. Folliculitis: This condition occurs when hair follicles become inflamed, often due to infection or irritation. If the bumps are red and tender, this could be a possibility, but it usually presents with more symptoms such as itching or discomfort.

4. Irritation or Allergic Reaction: Sometimes, irritation from soaps, lotions, or even friction from clothing can cause small bumps or redness. If you've recently changed any personal care products, consider reverting to your previous products to see if the symptoms improve.

5. Sexually Transmitted Infections (STIs): While you mentioned that you have not engaged in sexual activity, it is still important to consider this possibility, especially if there is any chance of exposure. STIs can present with various symptoms, including bumps, sores, or changes in the skin. If you have any concerns about potential exposure, it is advisable to get tested.

6. Other Dermatological Conditions: Conditions such as lichen planus or psoriasis can also cause changes in the skin, including bumps or discoloration. These conditions may require a dermatologist's evaluation for proper diagnosis and treatment.

Given that the bumps are not itchy or painful, this is a reassuring sign. However, it is crucial to monitor the situation. If the bumps change in appearance, increase in number, or if you develop any additional symptoms (such as pain, itching, or discharge), it would be wise to consult a healthcare professional for a thorough evaluation.

In the meantime, here are some general recommendations:
- Maintain Hygiene: Keep the area clean and dry. Use mild, unscented soap and avoid harsh chemicals that could irritate the skin.

- Avoid Irritants: If you suspect that a product may be causing irritation, discontinue its use.

- Monitor Symptoms: Keep an eye on the bumps for any changes. If they persist or worsen, seek medical advice.

In conclusion, while the small white and red bumps you are experiencing may not be a cause for alarm, it is always best to err on the side of caution. If you remain concerned or if any new symptoms arise, do not hesitate to reach out to a healthcare provider for further evaluation and peace of mind.
