Menstrual bleeding?
I have given birth to two children and currently have an intrauterine device (IUD) for contraception.
After my menstrual period ends, I experience bleeding again the next day, which can last from one day to three to five days.
The color of the blood varies, sometimes bright red and other times dark brown.
I have undergone examinations at the obstetrics and gynecology department, and everything appears normal.
Why do I experience this situation every month? What could be the issue, doctor?

Hello: If your menstrual cycle is regular, without dysmenorrhea or excessive bleeding, and your gynecological examination results are normal, it is still recommended to undergo a Pap smear.
Regular gynecological check-ups are advisable, and any explanations regarding your condition should be discussed with the physician conducting your examinations.
Thank you.

Irregular post-menstrual bleeding after the insertion of an intrauterine device (IUD) is a common concern among women. The IUD is a highly effective form of contraception, but it can also lead to various side effects, including changes in menstrual bleeding patterns. In your case, experiencing bleeding shortly after your menstrual period has ended can be attributed to several factors related to the IUD and your individual physiology.
Firstly, it is important to understand that the IUD can cause changes in the endometrial lining, which may lead to irregular bleeding. The hormonal IUD, for example, releases progestin, which can thin the uterine lining over time. This thinning can result in lighter periods, but it can also cause spotting or bleeding between periods, especially in the initial months after insertion. This is often referred to as "breakthrough bleeding."
In your situation, where you have given birth to two children and are now using an IUD, your body may still be adjusting to the device. The bleeding you describe—occurring one day to several days after your period—could be a normal response as your body adapts to the IUD. The color variations you mentioned, from bright red to dark brown, are also typical. Bright red blood usually indicates fresh bleeding, while dark brown blood often signifies older blood that has taken longer to exit the body.
It is reassuring that you have consulted with a gynecologist and that your examinations have returned normal results. This suggests that there are no underlying issues such as infections, fibroids, or polyps that could be causing the irregular bleeding. However, if the bleeding persists or worsens, or if you experience additional symptoms such as severe pain, heavy bleeding, or fever, it is crucial to seek further medical evaluation.
Another factor to consider is the timing of your IUD insertion. If the IUD was placed during your menstrual cycle, it might have disrupted your normal bleeding pattern. Additionally, hormonal fluctuations related to your menstrual cycle can also contribute to irregular bleeding.
In terms of management, if the bleeding is not heavy and does not interfere significantly with your daily life, it may be best to monitor the situation for a few more cycles. Many women find that their bleeding patterns stabilize after several months of IUD use. However, if the irregular bleeding continues beyond six months or if it becomes bothersome, discussing alternative contraceptive options with your healthcare provider may be warranted.
In summary, irregular post-menstrual bleeding after IUD insertion can be a common experience due to hormonal changes and the body's adjustment to the device. While it is often benign, ongoing communication with your healthcare provider is essential to ensure that your contraceptive method is suitable for you and to rule out any potential complications. If you have any concerns or if your symptoms change, do not hesitate to reach out for further evaluation and guidance.
