There are small bumps on the elbows (left and right)?

Hello Dr.
Kuo: I have developed small bumps on my elbows, which resemble mosquito bites, but they are not red or swollen.
It appears that small growths are forming on the surface of my skin.
They become very itchy, especially at night, and I can't help but scratch them, which leads to broken skin and small red wounds.
What could this symptom be? Is it possible that there is an issue with my immune system causing this skin condition? Thank you!

H.Y., 30~39 year old female. Ask Date: 2011/06/30

There are quite a few possibilities.
Based on your description, it seems to be urticaria, but it's difficult to make an accurate diagnosis without seeing the actual condition.
Please visit a dermatology department at a major hospital or a clinic for evaluation.

Dear Patient,
Thank you for reaching out with your concerns regarding the bumps on your elbows and the associated itching. Based on your description, it sounds like you may be experiencing a condition known as keratosis pilaris, which is characterized by small, rough bumps on the skin, often resembling goosebumps. This condition is quite common and usually occurs on the upper arms, thighs, cheeks, and sometimes on the elbows. It is caused by the buildup of keratin, a protein that protects the skin from infections and other harmful substances.
The itching you are experiencing, especially at night, could be attributed to several factors. First, dry skin is a common cause of itchiness, particularly in colder climates where the air is dry. When the skin loses moisture, it can become itchy and irritated, leading to scratching, which can further damage the skin and cause redness and small wounds.
Another possibility is that you may be experiencing an allergic reaction or irritation from something that comes into contact with your skin, such as soaps, lotions, or even fabrics. If you have recently changed any products or if you are using new detergents, this could be a contributing factor.

In terms of your immune system, while it is always good to be aware of your overall health, the symptoms you describe do not necessarily indicate a systemic immune issue. Conditions like keratosis pilaris are generally localized and not a sign of a broader immune dysfunction. However, if you have other symptoms or if the condition worsens, it would be wise to consult a healthcare professional for a thorough evaluation.

For treatment, here are some recommendations:
1. Moisturizing: Regularly apply a good moisturizer to your elbows and other affected areas. Look for creams that contain urea, lactic acid, or alpha-hydroxy acids, as these ingredients can help exfoliate the skin and reduce the buildup of keratin.

2. Gentle Exfoliation: Consider using a gentle exfoliating scrub or a chemical exfoliant containing salicylic acid or glycolic acid. This can help remove dead skin cells and prevent the bumps from becoming more pronounced.

3. Avoid Scratching: As tempting as it may be, try to avoid scratching the affected areas. Scratching can lead to further irritation and potential infections. Keeping your nails trimmed and using cold compresses can help alleviate the itch.

4. Topical Treatments: If the itching is severe, over-the-counter hydrocortisone cream may provide temporary relief. However, it’s important to use it sparingly and not for prolonged periods without consulting a doctor.

5. Consult a Dermatologist: If your symptoms persist or worsen, it would be beneficial to see a dermatologist. They can provide a definitive diagnosis and may prescribe stronger topical treatments or other therapies if necessary.

In conclusion, while the bumps on your elbows and the associated itching can be bothersome, they are often manageable with proper care. Keeping your skin moisturized and avoiding irritants will be key in managing your symptoms. If you have any further questions or if your condition changes, please do not hesitate to seek medical advice.

Best wishes for your skin health!
