Abnormal redness, swelling, and peeling of the lips?

Hello Doctor: I have a tendency for dry and chapped lips, so I often use commercially available lip balms without any issues.
However, in early May, my lower lip became persistently red and swollen without any peeling.
I consulted two or three dermatologists and took medications as well as applied ointments, but there seemed to be no signs of improvement; in fact, it became more swollen with each application.
Recently, after I stopped using the ointment, the redness and swelling gradually subsided, but peeling began.
When I applied Vaseline, my lips became red, swollen, and irritated.
What type of ointment should I use to prevent my lips from cracking? Is there a possibility of nutritional deficiencies? I would appreciate your guidance.

Drink plenty of water and avoid spicy and irritating foods.
Topical ointments can only be prescribed after a consultation, so I cannot recommend any.
Supplementing with B vitamins and vitamin C should be helpful.

More Info

Lip redness, swelling, and peeling can be quite distressing, and understanding the underlying causes is essential for effective treatment. Based on your description, it seems you have been dealing with chronic lip dryness and cracking, which is common for many individuals. However, the recent onset of redness and swelling without peeling is a different issue that may require a more nuanced approach.

Potential Causes
1. Allergic Reactions: One of the most common causes of lip swelling and redness is an allergic reaction to a product you may have used, such as lip balms, cosmetics, or even certain foods. Ingredients like fragrances, preservatives, or certain oils can trigger contact dermatitis, leading to inflammation.

2. Irritant Dermatitis: This occurs when your lips come into contact with irritating substances. This could be from environmental factors (like wind or sun exposure) or from products that are too harsh for sensitive skin.

3. Infections: Sometimes, infections (viral, bacterial, or fungal) can cause symptoms like swelling and redness. Herpes simplex virus, for instance, can lead to painful sores and swelling.

4. Nutritional Deficiencies: Deficiencies in certain vitamins, particularly B vitamins (like riboflavin and niacin) and vitamin C, can lead to dry, cracked lips. Ensuring a balanced diet can help mitigate these issues.

5. Underlying Skin Conditions: Conditions such as eczema or psoriasis can also affect the lips, causing redness and peeling.

Treatment Options
1. Identify and Avoid Triggers: If you suspect that a particular product is causing your symptoms, it’s crucial to stop using it. Consider switching to hypoallergenic products that are free from fragrances and preservatives.

2. Moisturization: While you mentioned that using Vaseline led to increased redness and swelling, it’s important to find a suitable lip balm that hydrates without causing irritation. Look for products containing natural ingredients like shea butter, coconut oil, or beeswax, which can provide moisture without harsh additives.

3. Hydrocortisone Cream: For short-term relief of inflammation, a mild over-the-counter hydrocortisone cream may help reduce redness and swelling. However, this should not be used long-term without consulting a healthcare provider.

4. Oral Supplements: If nutritional deficiencies are suspected, consider discussing with your healthcare provider about taking supplements or adjusting your diet to include more fruits, vegetables, and whole grains.

5. Consult a Dermatologist: Since you have already seen multiple dermatologists without improvement, it may be beneficial to seek a second opinion or ask for a referral to a specialist in dermatology who focuses on lip conditions. They may perform patch testing to identify any allergies or recommend a biopsy if a more serious condition is suspected.

Conclusion
In summary, lip redness, swelling, and peeling can stem from various causes, including allergic reactions, irritants, infections, nutritional deficiencies, or underlying skin conditions. It’s essential to identify the root cause to tailor an effective treatment plan. Avoiding known irritants, using suitable moisturizers, and possibly consulting a specialist can help you manage and alleviate your symptoms effectively. If symptoms persist or worsen, do not hesitate to seek further medical advice.
