Should I seek medical attention?

Dear Dr.
Zhang: Due to poor gynecological conditions (previously had a chocolate cyst treated with traditional surgery and adhesions treated with laparoscopy), I went to the hospital for ovulation induction injections and followed the doctor's advice to have intercourse, take hormones, and use progesterone suppositories.
About 10 days after ovulation, I had faint positive lines on the pregnancy test (using hormone and progesterone).
On the 12th day, I experienced spotting, and by the 14th day, it felt like my period started (the pregnancy test showed a negative result).
By the 16th day, my menstruation ended.
Should I seek medical attention again, or is this simply a case of natural miscarriage due to poor embryo quality?

Pang Pang, 30~39 year old female. Ask Date: 2011/09/21

It should belong to the latter.

When it comes to gynecological concerns, especially for individuals with a history of conditions such as endometriosis or ovarian cysts, it is crucial to pay attention to any unusual symptoms or changes in your menstrual cycle. Given your history of chocolate cysts and adhesions, along with the recent hormonal treatments you have undergone, it is understandable to feel concerned about your current situation.

From your description, it appears that you experienced a faint positive pregnancy test followed by spotting and then what seems to be an early menstrual period. This pattern can be indicative of several scenarios, including a chemical pregnancy, which is a very early miscarriage that occurs shortly after implantation. In a chemical pregnancy, the embryo may not develop properly, leading to a drop in hormone levels and the onset of menstruation.

Given your medical history and the recent hormonal treatments, it is advisable to seek medical attention. Here are several reasons why:
1. Monitoring Hormonal Levels: Since you have been on hormonal treatments, it is essential to monitor your hormone levels to ensure they are within the expected range. This can help determine if your body is responding appropriately to the medications.

2. Evaluating for Complications: Your history of chocolate cysts and adhesions can complicate your reproductive health. It is important to rule out any potential complications, such as ectopic pregnancy or retained tissue from a miscarriage, which could lead to further health issues if left untreated.

3. Understanding Your Cycle: The changes in your menstrual cycle, including the spotting and the timing of your period, warrant further investigation. A healthcare provider can perform an ultrasound or other diagnostic tests to assess the state of your ovaries and uterus, ensuring that there are no underlying issues.

4. Emotional Support: Experiencing a potential pregnancy loss can be emotionally taxing. Seeking medical attention can provide you with the necessary support and guidance to navigate this challenging time.

5. Future Fertility Considerations: If you are planning to conceive in the future, understanding your current reproductive health is crucial. A healthcare provider can offer insights into your fertility status and discuss any necessary interventions or treatments to optimize your chances of a successful pregnancy.

In conclusion, while it is possible that what you are experiencing is a natural response to hormonal changes or a very early pregnancy loss, it is always best to err on the side of caution, especially with your medical history. Scheduling an appointment with your gynecologist or a reproductive endocrinologist can provide you with the clarity and care you need. They can perform the necessary evaluations and offer personalized recommendations based on your unique situation. Remember, your health and well-being should always be a priority, and seeking medical advice is a proactive step in managing your reproductive health.
