Cleaning the foreskin?
Hello doctor, (1) the opening at the top of my urethra is a pinkish-red color, which is slightly different from the surrounding area, but it doesn't itch or hurt.
Is this normal? (2) When cleaning the glans, should I pull it all the way back or just to the coronal sulcus? (3) My scrotal skin is itchy (I wash it every day).
Can I apply eczema cream? Or could you please tell me what medication I can use?

Oh my god: Hello! 1.
It's uncertain.
2.
If possible, removing everything would be better for cleaning; it depends on personal habits, as long as it can be cleaned.
3.
Do not use soap or any cleaning products; alternatively, you can apply petroleum jelly.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

When it comes to penile hygiene, it is essential to understand the anatomy and the appropriate care practices to maintain health and prevent potential issues. Let's address your concerns one by one.
1. Color of the Urethral Opening: The urethral opening, or meatus, can vary in color among individuals. A pinkish or slightly red hue can be normal, especially if there are no accompanying symptoms such as itching, pain, or discharge. Variations in color can be due to factors like blood flow, skin type, and hygiene practices. However, if you notice any changes in color, texture, or if you develop symptoms, it is advisable to consult a healthcare professional for a thorough evaluation.
2. Cleaning the Glans: When cleaning the glans (the head of the penis), it is important to retract the foreskin (if uncircumcised) to expose the glans fully. You should clean the area gently, ensuring to reach the coronal sulcus (the groove where the glans meets the shaft) as this area can accumulate smegma, a substance that can lead to irritation or infection if not cleaned properly. Use warm water and mild soap, avoiding harsh chemicals or fragrances that can irritate the sensitive skin. It is crucial to rinse thoroughly to remove any soap residue.
3. Itching of the Scrotal Skin: Itching in the scrotal area can be caused by various factors, including irritation, fungal infections, or skin conditions like eczema. If you are experiencing persistent itching despite regular washing, it may be beneficial to avoid using any over-the-counter creams or ointments without consulting a healthcare provider first. While some eczema creams may provide relief, they can also cause further irritation if not appropriate for the area. A healthcare professional can recommend suitable treatments, such as antifungal creams if a fungal infection is suspected or a prescription-strength topical steroid for eczema.
In addition to addressing your specific questions, here are some general tips for maintaining penile hygiene:
- Regular Cleaning: Ensure to wash the genital area daily with warm water. If uncircumcised, retract the foreskin during washing to clean underneath thoroughly.
- Drying: After washing, make sure to dry the area well, as moisture can promote fungal growth.
- Avoid Irritants: Steer clear of scented soaps, lotions, or powders in the genital area, as these can lead to irritation.
- Wear Breathable Fabrics: Opt for loose-fitting underwear made from natural fibers like cotton to reduce moisture buildup and irritation.
- Monitor Changes: Keep an eye on any changes in color, texture, or symptoms such as itching, pain, or discharge. If you notice anything unusual, seek medical advice promptly.
In conclusion, maintaining proper penile hygiene is crucial for overall health. If you have any ongoing concerns or if symptoms persist, it is always best to consult with a healthcare professional who can provide personalized advice and treatment options.
