Inflammation and pain following a rash on the glans penis?
Hello Dr.
Hsu, recently I developed a white ulcer on the glans, which later turned into small red spots (similar to blisters).
After visiting a dermatologist, I was prescribed a topical medication.
After a couple of days, the skin on the glans appeared normal, but afterward, I experienced swelling and pain during erections.
Light pressure on the glans causes slight pain in the center.
A few days later, I consulted a urologist, who diagnosed me with glans inflammation and prescribed Diprogenta ointment, which seems primarily intended for treating dermatitis.
I mentioned the pain in the glans to the doctor, who said it should improve in a few days.
However, after using the ointment for four days, I still feel no improvement; I continue to experience pain during erections or when lightly pressing on the glans.
However, urination and other functions are completely normal, with no pain or unusual discharge.
I would like to ask Dr.
Hsu if I should observe for a few more days? Is Diprogenta only effective for skin injuries? Could this be a sign of another condition? Thank you.

After inflammation, there may be some tissue damage that requires time to fully heal.
During this period, local heat application can be beneficial in promoting blood circulation.
If new symptoms arise, it is important to seek medical attention promptly.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses should only be used as a reference; if necessary, it is important to visit a healthcare facility.

Understanding glans inflammation, also known as balanitis, is crucial for effective management and treatment. Balanitis can be caused by a variety of factors, including infections, irritations, and underlying medical conditions. Here’s a comprehensive overview of the causes, symptoms, and treatment options for glans inflammation.
Causes of Glans Inflammation
1. Infections: Bacterial, fungal, or viral infections can lead to inflammation of the glans. Common culprits include Candida (yeast infections), sexually transmitted infections (STIs) such as herpes or gonorrhea, and bacterial infections.
2. Irritation: Irritation from soaps, lotions, or other personal care products can cause inflammation. Additionally, friction from sexual activity or tight clothing can contribute to irritation.
3. Poor Hygiene: Inadequate cleaning of the genital area can lead to the accumulation of smegma, which can irritate the glans and lead to inflammation.
4. Skin Conditions: Conditions such as psoriasis, eczema, or lichen sclerosus can also cause inflammation of the glans.
5. Allergic Reactions: Allergies to latex (from condoms) or other materials can lead to inflammation.
Symptoms of Glans Inflammation
Symptoms of glans inflammation can vary but often include:
- Redness and swelling of the glans
- Pain or tenderness, especially during erection or pressure
- Itching or burning sensation
- Discharge or unusual odor (in cases of infection)
- Ulcerations or lesions on the glans
Treatment Options
The treatment for glans inflammation largely depends on the underlying cause. Here are some common approaches:
1. Topical Medications: As prescribed by your urologist, medications like Diprogenta (which contains betamethasone and gentamicin) can help reduce inflammation and treat infections. Betamethasone is a corticosteroid that reduces inflammation, while gentamicin is an antibiotic that can treat bacterial infections.
2. Antifungal Creams: If a fungal infection is suspected, antifungal creams may be prescribed.
3. Good Hygiene Practices: Maintaining proper hygiene is essential. Gently cleaning the area with mild soap and water can help prevent irritation and infection.
4. Avoiding Irritants: Identifying and avoiding potential irritants, such as certain soaps or personal care products, can help alleviate symptoms.
5. Oral Medications: In some cases, oral antibiotics or antifungals may be necessary, especially if the infection is severe or widespread.
Monitoring and Follow-Up
Given your situation, where you have experienced initial improvement followed by persistent pain during erection or pressure, it is important to continue monitoring your symptoms. If you do not see improvement after a few more days of treatment with Diprogenta, or if your symptoms worsen, it would be advisable to follow up with your healthcare provider. They may consider additional diagnostic tests to rule out other conditions, such as STIs or other skin disorders.
Conclusion
While Diprogenta is effective for treating certain types of skin inflammation and infections, it is essential to ensure that the underlying cause of your symptoms is adequately addressed. If you continue to experience pain or discomfort, further evaluation may be necessary to determine if there are other underlying issues that need to be treated. Always communicate openly with your healthcare provider about your symptoms and any concerns you may have regarding your treatment plan.
