Red spots on the glans?

Hello Doctor: Today while taking a shower, I noticed that my glans was itchy.
When I pulled back the foreskin, I found 2 or 3 small raised red spots on the upper right side of the glans, varying in size but around 1mm each.
Sometimes they itch, sometimes they don't.
There are also some red areas on the foreskin (the redness has faded the next day), but the small red spots have not disappeared.
What should I do? Is this a sexually transmitted infection? Will it resolve on its own? I have not had sexual intercourse.

Beijue, 10~19 year old female. Ask Date: 2012/05/31

Dr. Li Yilun reply Urology

Sexually transmitted diseases (STDs) refer to infections transmitted through sexual activity.
If there is no sexual contact, the likelihood of transmission is obviously nonexistent.
Some skin lesions on the glans may be manifestations of dermatological conditions such as dermatitis.
However, it is important to emphasize that if there are any concerns, it is advisable to consult a dermatologist or urologist.
Physicians often need to perform visual examinations, palpation, and take a detailed medical history to establish a diagnosis and determine the appropriate treatment.

When it comes to red bumps on the glans (the head of the penis), it's important to approach the situation with care and understanding. The symptoms you describe—itching, small red bumps, and some redness of the foreskin—can be concerning, but they are not necessarily indicative of a sexually transmitted infection (STI), especially since you mentioned that you have not engaged in sexual intercourse.

There are several potential causes for the symptoms you are experiencing:
1. Irritation or Allergic Reaction: Sometimes, irritation from soaps, lotions, or even laundry detergents can cause localized redness and bumps. If you have recently changed any personal care products, this could be a factor.

2. Folliculitis: This is an inflammation of the hair follicles, which can occur anywhere on the body, including the genital area. It can present as small red bumps and may be itchy.

3. Pearly Penile Papules: These are small, harmless bumps that can appear around the glans and are often mistaken for something more serious. They are a normal anatomical variation and do not require treatment.

4. Balanitis: This is an inflammation of the glans, often caused by poor hygiene, infections, or irritants. Symptoms can include redness, itching, and bumps.

5. Infections: While STIs are a possibility, other infections such as yeast infections or bacterial infections can also cause similar symptoms.
Given that the redness on the foreskin has subsided but the small red bumps remain, it may indicate that the initial irritation has improved, but the bumps could be a separate issue.

What Should You Do?
1. Avoid Irritants: If you suspect that soaps or lotions may be causing irritation, consider switching to mild, unscented products.
2. Maintain Hygiene: Ensure that you are keeping the area clean and dry. Gentle washing with water and mild soap can help.

3. Monitor Symptoms: Keep an eye on the bumps. If they change in appearance, increase in number, or if you develop additional symptoms (such as pain, discharge, or fever), it is important to seek medical attention.

4. Consult a Healthcare Provider: Since you are experiencing discomfort and have concerns about the possibility of an STI, it would be wise to consult a urologist or a healthcare provider. They can perform a physical examination and may recommend tests to rule out infections or other conditions.

5. Avoid Self-Diagnosis: While it’s natural to seek answers online, self-diagnosis can lead to unnecessary anxiety or inappropriate treatment. A healthcare professional can provide a proper diagnosis and treatment plan tailored to your specific situation.

Conclusion
In summary, while the symptoms you are experiencing can be concerning, they are not definitively indicative of a sexually transmitted infection, especially given your sexual history. The best course of action is to maintain good hygiene, avoid potential irritants, and consult a healthcare provider for a thorough evaluation. They can provide you with the reassurance and treatment you may need. Remember, early intervention can often lead to better outcomes, so don’t hesitate to seek help.
