There are small red spots on the glans?

Hello doctor, I have small red bumps on my glans that are slightly raised.
They are not painful or itchy and have been present for several months.
I masturbate about twice a week and have never had sexual intercourse.
Initially, there were only one or two bumps, and since they are not painful or itchy, I thought they would resolve on their own.
However, they have increased in number.
Normally, they are small, but they become larger and more prominent during an erection.
What could this be?

Xiao Wei, 20~29 year old female. Ask Date: 2015/12/13

Dr. Xu Weikai reply Urology

Xiao Wei: Hello, I recommend seeking medical attention as soon as possible.
It could be due to localized inflammation, infection, or vascular-related issues.
It may also be a normal condition or a result of excessive or incorrect cleaning practices.
(For the correct method of cleaning the glans, please refer to the advice I previously provided to online users.) If there are any unclear points, feel free to ask again, and we can discuss and research further.

Red bumps on the glans can be concerning, especially when they persist for several months without any pain or itching. Based on your description, there are a few possibilities to consider, but it’s essential to consult a healthcare professional for an accurate diagnosis and appropriate treatment.

One common cause of red bumps on the glans is Pearly Penile Papules. These are small, harmless bumps that are often found around the corona of the glans and are completely normal anatomical variations. They are not sexually transmitted and do not require treatment unless they cause psychological distress.

Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the glans and are caused by enlarged sebaceous glands. Like pearly penile papules, they are benign and do not require treatment.

However, if the bumps are new, changing in appearance, or if you notice any other symptoms such as discharge, pain, or changes in urination, it could indicate a different condition, such as:
1. Folliculitis: This is an inflammation of the hair follicles that can lead to small red bumps. It can occur due to irritation, infection, or blockage of the follicles.

2. Genital Warts: Caused by certain strains of the human papillomavirus (HPV), these can appear as small, flesh-colored or red bumps. They may not be painful but can spread through sexual contact.

3. Contact Dermatitis: This can occur due to irritation from soaps, lotions, or other products that come into contact with the genital area.

4. Balanitis: This is an inflammation of the glans, which can be caused by infections, poor hygiene, or skin conditions. Symptoms may include redness, swelling, and sometimes discharge.

Given that you mentioned these bumps have been present for several months and have increased in number, it is advisable to seek medical attention. A urologist or dermatologist can perform a physical examination and may recommend tests to determine the exact cause of the bumps. They may also provide treatment options if necessary.

In the meantime, here are some general recommendations:
- Avoid Irritants: Be cautious with soaps, lotions, or any products that may irritate the area. Opt for gentle, fragrance-free products.

- Maintain Hygiene: Keep the area clean and dry. Regular washing with mild soap and water can help prevent infections.

- Avoid Sexual Activity: Until you have a clear diagnosis, it may be wise to avoid sexual activity to prevent potential transmission of any underlying conditions.

- Monitor Changes: Keep an eye on the bumps for any changes in size, color, or symptoms. Documenting these changes can be helpful for your healthcare provider.

In conclusion, while many causes of red bumps on the glans are benign, it is essential to consult a healthcare professional for a proper evaluation. Early diagnosis and treatment can help alleviate any concerns and ensure your urological health is maintained.
