Effective Use of Azelaic Acid: Dosage, Duration, and Moisturizing Tips - Dermatology

Regarding rhododendron acid?

Hello, I am currently using azelaic acid at a concentration of 20%, applying it once in the morning and once at night.
I apply it to my entire face, so I would like to know how much product I should use for each application to be effective.
How long will it take to see a reduction in acne scars? After applying the treatment, can I use a moisturizer? I find that my face tends to get oily after applying the medication, but I'm concerned that using a moisturizer might affect its efficacy.

Dr. Wu Yuxin reply Dermatology

As long as there is no allergic reaction, it can continue to be used.
The general guideline for application is to apply it twice until the ointment is no longer visible; there is no need to apply excessively.
Fading of acne scars typically takes about 1-2 months, and it also depends on the depth of the scar color.
After application, it is still possible to use a moisturizing lotion.

More Info

Azelaic acid is a versatile topical treatment that is commonly used for various skin conditions, including acne and rosacea. It is known for its anti-inflammatory, antibacterial, and keratolytic properties, making it effective in reducing acne lesions and improving skin texture. Here’s a comprehensive overview of its effective use, dosage, duration, and moisturizing tips.

Effective Dosage and Application
When using azelaic acid, especially at a concentration of 20%, the typical recommendation is to apply a pea-sized amount to the affected areas of the skin. This amount is generally sufficient to cover the entire face without over-applying. Applying too much can lead to increased irritation, which is counterproductive. It’s crucial to ensure that the skin is clean and dry before application to maximize absorption and effectiveness.

Duration for Results
The duration required to see noticeable improvements in acne scars or hyperpigmentation can vary significantly from person to person. Generally, it may take about 4 to 12 weeks of consistent use to see significant results. Some individuals may notice improvements sooner, while others may require a longer duration. It’s important to maintain realistic expectations and understand that while azelaic acid can help fade scars, complete removal may not be achievable for everyone.

Incorporating Moisturizers
After applying azelaic acid, it is advisable to follow up with a moisturizer. While it’s understandable to be concerned about the potential for increased oiliness, using a suitable moisturizer can actually enhance the overall effectiveness of your treatment. A lightweight, non-comedogenic moisturizer can help maintain skin hydration without clogging pores. Look for products labeled as "oil-free" or "non-acnegenic" to minimize the risk of exacerbating acne.

Managing Oily Skin
If you find that your skin becomes excessively oily after applying azelaic acid, consider the following tips:
1. Choose the Right Moisturizer: Opt for gel-based or water-based moisturizers that provide hydration without adding excess oil. Ingredients like hyaluronic acid can be beneficial as they hydrate the skin without being heavy.

2. Adjust Application Frequency: If you experience significant oiliness, you might consider applying azelaic acid once a day instead of twice, especially if your skin is sensitive or reactive.

3. Monitor Skin Response: Pay attention to how your skin reacts over time. If you notice excessive irritation or oiliness, consult with a dermatologist who can provide personalized recommendations.

4. Use Oil-Absorbing Products: If oiliness is a persistent issue, consider incorporating oil-absorbing products, such as blotting papers or mattifying primers, into your routine.

Conclusion
In summary, azelaic acid is an effective treatment for acne and post-inflammatory hyperpigmentation when used correctly. A pea-sized amount applied twice daily is generally sufficient, and results can typically be seen within 4 to 12 weeks. Incorporating a lightweight moisturizer after application can help manage oiliness and maintain skin hydration. Always consult with a dermatologist if you have specific concerns or if your skin does not respond as expected. They can provide tailored advice and adjust your treatment plan as necessary.
