Penile Curvature: Causes and Effects on Sexual Health - Urology

Regarding the issue of penile curvature?

Hello doctor, my penis has a downward curvature of about 20 to 30 degrees.
Recently, I have had sexual experiences with my girlfriend, and while there is no discomfort during penetration, she feels pain when I first enter.
However, during subsequent rounds, she experiences some pain in her lower region, and after we finish, she has a burning sensation when urinating, and last time there was even some bleeding.
I would like to know if this is related to my penile curvature.
My girlfriend has not had as many sexual encounters with her previous boyfriend (they only did it once at most), so I am unsure if her discomfort is due to my penile curvature or if it is because we are having intercourse multiple times in a short period.
Could you please clarify this for me? Thank you.

Hello: The medical term for penile curvature is "penile tunica albuginea anomaly." The penis relies on the internal two "corpora cavernosa" to become engorged for an erection.
During male puberty, the penis develops rapidly, and if the growth of the tunica albuginea is uneven in any direction, when the corpora cavernosa engorge, the differing degrees of stretch in the tunica can result in curvature.
When the angle of curvature exceeds 30 degrees, it may be difficult to penetrate the vagina during intercourse, and most importantly, it can lead to "bending" during intercourse, which may cause penile injury or even fracture (i.e., tunica rupture), thus requiring active treatment.
The only treatment method is surgical correction.
The optimal timing for surgery is after reaching adulthood, as puberty is still a period of development, and early surgery may lead to overcorrection or recurrence.
There are several surgical methods, all based on the principle of "shortening the longer side," where the tunica on the longer side is sutured to equal the length of the opposite side.
Techniques include the more traditional Nesbit procedure (where part of the tunica is excised and then sutured) and tunica plication.
If intercourse causes discomfort or pain to the partner, surgical treatment is advisable, and it is recommended to consult a specialist for a detailed evaluation.
Thank you!

Understanding penile curvature and its potential effects on sexual health is an important topic, especially when it comes to ensuring both partners' comfort and safety during sexual activity. Penile curvature, which can be a natural anatomical variation, may be caused by several factors, including congenital conditions, Peyronie's disease, or even trauma. In your case, a curvature of 20 to 30 degrees downward is not uncommon, and many men live with such variations without significant issues.

However, the discomfort your girlfriend experiences during intercourse, particularly after multiple rounds, raises some important considerations. It's crucial to understand that while penile curvature itself may not directly cause pain, it can affect the angle and depth of penetration, which might lead to discomfort for your partner. The pain she feels, especially if it is localized to her lower abdomen or pelvic area, could be due to the angle at which penetration occurs, potentially causing pressure on sensitive areas.

Moreover, the burning sensation during urination and the occurrence of bleeding after intercourse are concerning symptoms that should not be overlooked. These symptoms could indicate irritation or trauma to the vaginal walls, especially if the intercourse is vigorous or if there is insufficient lubrication. It's essential to ensure that both partners are adequately aroused and that sufficient lubrication is used to minimize friction, which can lead to micro-tears in the vaginal tissue.

Given that your girlfriend has not experienced such frequency of intercourse with previous partners, her body may need time to adjust to the increased activity. The combination of your penile curvature and the frequency of intercourse may be contributing to her discomfort. It’s also possible that her body is reacting to the increased friction or pressure, leading to inflammation or even minor injuries.

To address these issues, consider the following recommendations:
1. Communication: Openly discuss with your girlfriend what feels comfortable and what does not. Adjusting positions may help alleviate discomfort. For instance, trying positions that allow for shallower penetration or adjusting the angle of entry may be beneficial.

2. Lubrication: Always use a high-quality lubricant during intercourse to reduce friction. This can help prevent irritation and make the experience more pleasurable for both partners.

3. Pacing: If you notice discomfort after multiple rounds, consider taking breaks between sessions or reducing the frequency to allow her body to recover.

4. Medical Consultation: Given the burning sensation during urination and the bleeding, it would be wise for both you and your girlfriend to consult a healthcare provider. They can assess for any underlying issues, such as infections or injuries, and provide appropriate treatment if necessary.

5. Positioning: Experimenting with different sexual positions can help find what works best for both of you. Some positions may allow for better angles that reduce discomfort.

In conclusion, while penile curvature itself may not be the sole cause of your girlfriend's discomfort, it can play a role in the overall experience. By addressing lubrication, communication, and pacing, you can work towards a more comfortable and enjoyable sexual experience. If symptoms persist, seeking medical advice is crucial to rule out any underlying health concerns.
