There are white bumps on the inner side of the foreskin?
Hello Doctor: I have noticed small white spots on the inner side of my foreskin for the past three to four years.
They are not painful or itchy, but recently I feel like there might be more of them (though I'm not entirely sure).
I would like to ask what this could be? (I have not engaged in sexual activity.) Could you please take a look? Thank you.

It appears to be a suspected sebaceous gland structure.
To confirm, a biopsy must be performed by the relevant specialty to determine the tissue architecture.

Understanding white bumps on the inner foreskin can be a source of concern for many individuals. These bumps, often described as small white spots or papules, can arise from various causes, and it is essential to differentiate between benign conditions and those that may require medical attention.
One common cause of white bumps on the inner foreskin is Pearly Penile Papules (PPP). These are small, dome-shaped, white or skin-colored bumps that typically appear around the corona of the glans (the head of the penis). PPP is a normal anatomical variant and is not associated with any disease or infection. They are harmless and do not require treatment unless they cause psychological distress.
Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the inner foreskin and other areas of the genital region. These spots are sebaceous glands that are not associated with hair follicles and are completely normal. Like PPP, Fordyce spots are benign and do not require any treatment.
Balanitis, an inflammation of the glans, can also lead to the appearance of white bumps or patches. This condition can be caused by poor hygiene, infections (such as yeast infections), or irritants. Symptoms may include redness, swelling, and discomfort. If you suspect balanitis, it is advisable to maintain good hygiene and consult a healthcare provider for appropriate treatment.
In some cases, genital warts caused by the human papillomavirus (HPV) can present as white or flesh-colored bumps on the genital area, including the inner foreskin. These warts can vary in size and may appear singularly or in clusters. If you notice any changes in the appearance of the bumps, or if they become painful, itchy, or bleed, it is crucial to seek medical evaluation.
It is also worth considering Lichen Sclerosus, a chronic inflammatory skin condition that can affect the genital area, leading to white patches or bumps. This condition may require treatment to manage symptoms and prevent complications.
Given that you mentioned these bumps have been present for several years and are not causing any pain or discomfort, it is likely that they are benign. However, the fact that you have noticed a potential increase in their number warrants a medical evaluation. A healthcare provider can perform a physical examination and, if necessary, conduct further tests to rule out any serious conditions.
In summary, while many causes of white bumps on the inner foreskin are benign, it is essential to monitor any changes in their appearance or associated symptoms. If you have concerns, especially since you mentioned not having engaged in sexual activity, it would be prudent to consult a healthcare professional for a thorough evaluation and peace of mind. Regular check-ups and open communication with your doctor can help ensure your genital health remains a priority.
