Phimosis: Concerns, Treatments, and Myths Explained - Urology

Phimosis issue

Hello doctor, I am currently 20 years old and have a problem with phimosis.
It seems to be complete phimosis because I cannot retract the foreskin at all; it is estimated to be about 4-5 cm when not erect and 9-10 cm when erect.
This seems quite small, although I can still masturbate.
However, if I engage in sexual intercourse in the future, I may not be able to do so if the foreskin cannot retract.
Therefore, I have considered circumcision, but it seems that many people today do not support it and even say that it may result in a smaller size due to retraction.
Additionally, I feel that after circumcision, the tightness around the glans might make erections more difficult.
I have also heard about the foreskin restoration technique.
Is it effective? Are these claims true? Thank you, doctor, for taking the time to answer.

Dr. Xu Weikai reply Urology

Xiao Li: Hello, circumcision is unrelated to penis size.
After circumcision, unless there is a specific condition that causes scar tissue contraction, the situation you mentioned will not occur.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info

Phimosis is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis). This can be a source of concern for many men, especially when it comes to sexual activity and hygiene. As you mentioned, you are currently 20 years old and experiencing what you believe to be complete phimosis. This means that you are unable to retract the foreskin at all, which can lead to difficulties during sexual activity and potential hygiene issues.

Concerns Related to Phimosis
1. Sexual Function: One of the primary concerns for men with phimosis is the ability to engage in sexual intercourse. If the foreskin cannot be retracted, it may cause discomfort or pain during sexual activity. In some cases, it may even prevent penetration altogether.

2. Hygiene: A non-retractable foreskin can make it difficult to maintain proper hygiene, leading to a buildup of smegma (a combination of dead skin cells and oils) under the foreskin. This can increase the risk of infections.

3. Psychological Impact: Many men may feel embarrassed or anxious about their condition, which can affect their self-esteem and relationships.

Treatment Options
1. Conservative Management: In some cases, gentle stretching exercises may help to gradually loosen the foreskin. This approach requires patience and consistency, and it is essential to do it gently to avoid pain or injury.

2. Topical Steroid Creams: Some healthcare providers may recommend the use of topical steroid creams to help soften the skin of the foreskin, making it easier to retract over time.

3. Circumcision: This is a surgical procedure where the foreskin is removed. Circumcision is often considered when conservative treatments are ineffective or if the phimosis is causing significant problems. It is a common procedure and is generally safe, but like any surgery, it carries risks.

4. Preputioplasty: This is a less invasive surgical option that involves making a small incision in the foreskin to allow for easier retraction without complete removal. This procedure can preserve the foreskin while alleviating the symptoms of phimosis.

5. Frenuloplasty: If the frenulum (the band of tissue on the underside of the penis) is too tight, this procedure can be performed to release the tension, allowing for easier retraction of the foreskin.

Myths and Misconceptions
1. Circumcision and Size: There is a common myth that circumcision will result in a smaller penis. However, the size of the penis is not affected by circumcision. The appearance may change slightly due to the removal of the foreskin, but the actual size remains the same.

2. Erectile Function: Concerns about decreased erectile function after circumcision are largely unfounded. Many studies have shown that circumcision does not negatively impact erectile function or sexual satisfaction.

3. Foreskin Restoration: Some individuals may consider foreskin restoration techniques, which involve using devices or methods to stretch the skin and create a new foreskin. However, these methods are often time-consuming and may not yield satisfactory results.

Conclusion
If you are experiencing phimosis and are concerned about its implications for your sexual health and hygiene, it is essential to consult with a healthcare provider. They can provide a thorough evaluation and discuss the best treatment options for your situation. It is also important to address any myths or misconceptions you may have about circumcision or other treatments, as understanding the facts can help you make an informed decision. Remember, you are not alone in this, and many men face similar challenges. Seeking professional advice is a positive step toward addressing your concerns.
