Erectile pain after penile curvature surgery?
Hello, Doctor.
Last week, I underwent surgery for penile curvature correction, specifically a tunica albuginea plication procedure.
However, that very night, I was awakened by pain due to nocturnal erections.
Since then, I have been waking up in pain almost every night, approximately every two hours, due to erections (especially painful when they are restricted).
This occurs about four to six times a night (of course, I have no issues while awake during the day).
I have read various articles online that mention the need to strictly limit nocturnal erections and sexual activity; I can manage the latter, but I am at a loss for the former.
I would like to ask if these frequent mild erections (which soften immediately after waking) could lead to suture failure? I try to keep my penis in an upright position to minimize pain during erections, but I am really worried about the sutures breaking or failing.
I am having a hard time sleeping every night and would greatly appreciate your clarification.
Thank you.

Tom: After penile curvature correction surgery, frequent erections can indeed damage the sutures, and in severe cases, it may even lead to rupture.
You may consult your surgeon, and if necessary, medications can be used to reduce the frequency of erections.
Additionally, bladder distension can also trigger erections, so reducing fluid intake day and night can help avoid this.
If there are any unclear points, feel free to ask again for further discussion and research.

Post-surgery pain, especially after a penile curvature correction surgery, can be a significant concern for many patients. It’s essential to understand the nature of the surgery and the healing process to manage discomfort effectively.
Firstly, it’s important to acknowledge that the procedure you underwent, typically involving techniques like the tunica albuginea plication or grafting, aims to correct the curvature of the penis. This surgery can lead to postoperative pain, particularly during erections, as the surgical site is healing. The pain you are experiencing during nocturnal erections is not uncommon and can be attributed to several factors.
After surgery, the body goes through a healing process that includes inflammation and tissue repair. During this time, the surgical site may be sensitive, and erections can cause discomfort due to the tension on the sutures and surrounding tissues. The fact that you are experiencing pain during erections, especially when they are obstructed, suggests that the surgical area is still quite sensitive and may not have fully healed yet.
Regarding your concern about the frequency of nocturnal erections and the potential for suture failure, it’s crucial to understand that while erections can cause discomfort, they are a natural physiological response. Nocturnal erections are common and occur during REM sleep. They are generally not harmful, but if they are causing significant pain, it may be worth discussing with your surgeon.
To manage the discomfort, consider the following strategies:
1. Pain Management: Over-the-counter pain relief medications, such as ibuprofen or acetaminophen, can help alleviate discomfort. However, consult your doctor before taking any medication to ensure it’s appropriate for your situation.
2. Cold Compress: Applying a cold compress to the area can help reduce swelling and numb the pain. Ensure that the compress is not applied directly to the skin to avoid frostbite.
3. Sleep Position: Adjusting your sleep position may help minimize discomfort. Some patients find that sleeping on their side or in a position that reduces pressure on the pelvic area can be beneficial.
4. Limit Stimulation: While it’s essential to allow the body to heal, try to avoid any activities that may lead to unwanted erections, such as sexual arousal or stimulation. This can help reduce the frequency of painful episodes.
5. Follow-Up with Your Surgeon: It’s crucial to maintain open communication with your healthcare provider. If the pain persists or worsens, or if you have concerns about the integrity of the surgical site, a follow-up appointment is necessary. Your surgeon can assess the healing process and provide tailored advice or interventions.
6. Physical Therapy: In some cases, pelvic floor physical therapy may be recommended to help manage pain and improve recovery. A physical therapist specializing in post-surgical recovery can provide exercises and techniques to alleviate discomfort.
In summary, while experiencing pain during nocturnal erections after penile curvature correction surgery is not uncommon, it is essential to manage this discomfort effectively. Maintaining open communication with your healthcare provider and following their recommendations will be key to ensuring a smooth recovery. If pain persists or if you have any concerns about your healing process, do not hesitate to seek medical advice. Your health and comfort are paramount, and addressing these issues early can help prevent complications and promote a successful recovery.
