After ligament tear..?

Hello, doctor! I'm too shy to seek medical attention, but I feel that the situation is serious, so I hope Dr.
Chou can give me some advice.
Two months ago, during a sexual encounter, I experienced a severe tear of the frenulum, resulting in significant bleeding and a wound approximately 1 cm long and 0.2 cm wide.
I managed to stop the bleeding at the time but did not seek medical care.
At that moment, there was a prominent vein on the lower left side of the frenulum that appeared to be engorged.
After the wound healed, my nightmare began...
Now, during an erection, I find it very easy to lose firmness.
Although I can still insert, it just doesn't feel right.
The main issue is that I ejaculate very easily, often before I even feel anything, which is completely different from before.
I can hardly engage in sexual activity anymore...
My girlfriend has gradually lost interest, and it’s almost like we’ve broken up...
I don’t know what to do anymore...
Thank you for your response.

I'm sorry to hear about your situation. It sounds very distressing, and it's understandable that you're seeking advice. Based on your description, it seems that you experienced a significant injury during sexual activity, which has led to ongoing issues with erectile function and premature ejaculation.

Firstly, it's important to recognize that trauma to the genital area, especially involving the ligaments and blood vessels, can have various effects on sexual function. The symptoms you're experiencing—difficulty maintaining an erection, changes in sensation, and premature ejaculation—could be related to several factors, including physical damage to the tissues, psychological impacts from the trauma, or a combination of both.

1. Physical Damage: The injury you described, particularly the tearing of ligaments and the presence of a hematoma (the "blue vein" you mentioned), could have caused some structural changes in the area. This might affect blood flow and nerve function, both of which are crucial for achieving and maintaining an erection. If the injury has led to scarring or changes in the anatomy, it could also contribute to the sensations you're experiencing during sexual activity.

2. Psychological Impact: Sexual trauma can have significant psychological effects. Anxiety about performance, fear of re-injury, or feelings of shame can all contribute to erectile dysfunction and changes in sexual response. It's not uncommon for individuals who have experienced trauma to develop anxiety around sexual activity, which can further exacerbate physical issues.

3. Seeking Medical Help: While it may feel daunting, I strongly encourage you to seek medical attention. A healthcare professional, particularly a urologist or a sexual health specialist, can provide a thorough evaluation. They may recommend imaging studies to assess any physical damage and discuss potential treatment options, which could include physical therapy, medication, or counseling.

4. Counseling and Support: In addition to physical treatment, consider speaking with a therapist who specializes in sexual health or trauma. They can help you work through any psychological barriers and develop coping strategies to improve your sexual function and relationship.

5. Communication with Your Partner: If you feel comfortable, try to communicate openly with your girlfriend about what you're experiencing. While it may be difficult, sharing your feelings and concerns can help her understand your situation better and may alleviate some of the pressure you feel during intimate moments.

6. Lifestyle Changes: In the meantime, focusing on overall health can be beneficial. Regular exercise, a balanced diet, and stress management techniques (like mindfulness or yoga) can improve your physical and mental well-being, which may positively impact your sexual health.

In summary, while your symptoms are concerning, they are not uncommon following a significant injury. Seeking professional help is crucial for both physical and psychological recovery. Remember, you are not alone in this, and there are resources available to help you navigate these challenges.
