Dry eyes upon waking?

Hello, Dr.
Gao: I apologize for the interruption during your busy schedule.
I would like to ask you the following questions:
1.
If I have had corneal abrasions in the past (approximately two occurrences this year, which have healed without scarring), could this make my eyes more prone to dryness? I had an examination the day before yesterday, and the doctor said I do not have dry eye syndrome.
However, why do I feel a bit of dryness in my eyes every morning upon waking up? (This occurs without the air conditioning on.) The sensation of dryness only happens in the morning; at other times, I do not experience it.
Could this be a psychological effect?

ANGELA, 20~29 year old female. Ask Date: 2012/11/09

Dr. Gao Pilin reply Ophthalmology

Hello: When sleeping, the eyelids are closed and do not blink, which prevents tears from spreading across the entire surface of the eye.
Sometimes, the eyelids do not close completely, leading to increased evaporation of moisture from the eyes, even without air conditioning.
If there is no dry eye syndrome, the feeling of dryness upon waking should improve with more frequent blinking.
If symptoms are severe, applying lubricating eye ointment before bed or using artificial tears upon waking may help alleviate the symptoms.
Thank you for your message.

More Info

Waking up with dry eyes is a common experience for many individuals and can be attributed to several physiological factors. In your case, you mentioned a history of corneal abrasions, which can indeed influence your eye health, but it is important to understand the broader context of why you may be experiencing dryness specifically in the morning.

1. Natural Tear Production: During sleep, the body’s production of tears decreases significantly. The tear film, which is essential for keeping the eyes moist and comfortable, is composed of three layers: lipid, aqueous, and mucin. When you sleep, the eyelids are closed, and the eyes are not blinking, which means that the tear film is not replenished. This can lead to a temporary state of dryness upon waking.
2. Environmental Factors: Even if you are not using air conditioning, other environmental factors can contribute to dryness. For instance, sleeping in a room with low humidity or exposure to drafts can exacerbate the feeling of dry eyes. Additionally, allergens or irritants in the environment can also affect your eyes overnight.

3. Corneal Health: You mentioned having experienced corneal abrasions in the past. While your doctor has indicated that you do not have dry eye syndrome, it is possible that the healing process of the cornea can lead to temporary sensations of dryness, especially if the corneal surface is still sensitive. The cornea has a high density of nerve endings, and any previous injury can make it more susceptible to sensations of discomfort or dryness.

4. Blinking and Eye Closure: During sleep, the eyelids do not blink, which is crucial for spreading tears across the surface of the eye. If your eyelids do not close completely (a condition known as lagophthalmos), this can lead to increased evaporation of tears and a feeling of dryness upon waking.

5. Psychological Factors: While it is less common, anxiety or stress can manifest in physical symptoms, including sensations of dryness or discomfort in the eyes. If you are particularly anxious about your eye health, this could potentially heighten your awareness of any discomfort you experience.

6. Hydration and Health: General hydration levels can also affect tear production. Ensure you are drinking enough water throughout the day, as dehydration can lead to reduced tear production.

In conclusion, while your experience of dry eyes upon waking is likely due to a combination of reduced tear production during sleep and environmental factors, it is always a good idea to consult with an eye care professional if you have ongoing concerns. They can provide a thorough examination to rule out any underlying conditions and suggest appropriate treatments, such as artificial tears or lifestyle modifications to help alleviate your symptoms.
If your symptoms persist or worsen, or if you experience additional symptoms such as redness, pain, or changes in vision, it is advisable to seek medical attention promptly. Regular check-ups, as you mentioned, are also a good practice to monitor your eye health, especially given your history of corneal issues.
