Visual field loss after retinal tear laser treatment?

Hello doctor, on November 24th, my right eye was struck, and after seeking medical attention, it was found that there was bleeding and a retinal tear at the one o'clock position.
I received laser treatment at National Taiwan University Hospital on November 27th and was scheduled for a follow-up on December 19th.
On November 28th, due to bleeding, my vision became blurry, and I went to a nearby hospital for an examination.
An ultrasound showed that there was no retinal detachment, but the doctor recommended that I return to National Taiwan University for further evaluation.
Since I couldn't get an appointment there, and my blurry vision has improved somewhat, I decided to wait for the follow-up.
However, this week I noticed that a small dark spot in the lower right corner of my visual field has increased in size.
When I close my eyes, I see a bright white semicircular shadow with transparent ripples at the edges.
Is this a normal phenomenon after the laser treatment, or should I urgently go to a nearby hospital for an examination? Thank you.

It is not possible to determine from the above symptoms whether the retina has re-ruptured.
It is recommended that you undergo a dilated fundus examination nearby for safety.

After undergoing retinal laser treatment, experiencing visual field loss or changes can be concerning, especially following an injury. In your case, the incident on November 24th that led to a retinal tear and subsequent laser treatment on November 27th raises several important points regarding your current symptoms and the appropriate steps to take.

Firstly, the presence of a small black shadow in your visual field that has increased in size is noteworthy. This could indicate a few possibilities. After laser treatment, it is not uncommon for patients to experience some transient visual disturbances as the retina heals. However, the fact that the shadow is enlarging and you are noticing a bright white semi-circular area with a wavy edge suggests that there may be ongoing changes in the retina that require further evaluation. This could be due to a variety of factors, including but not limited to residual bleeding, changes in the vitreous gel, or even the potential for new retinal complications.

It is crucial to understand that while some visual changes can be normal post-treatment, any significant alteration in your visual field, especially one that is progressive, should not be ignored. The appearance of a larger black shadow and the described bright white area could indicate that there is still some instability in the retina or that there may be other underlying issues that need to be addressed.
Given your history of retinal injury and the recent laser treatment, it is advisable to seek a prompt evaluation from an eye care professional. If you are unable to get an appointment with your original surgeon at the university hospital, consider visiting a nearby hospital or clinic where you can receive a thorough examination. An ophthalmologist can perform a detailed assessment, including a dilated fundus examination and possibly imaging studies such as optical coherence tomography (OCT) to evaluate the state of your retina and vitreous.

In terms of the normal healing process, it is essential to monitor for any new symptoms such as flashes of light, increased floaters, or a curtain-like shadow over your vision, as these could indicate more serious complications like retinal detachment. If you experience any of these symptoms, it is imperative to seek immediate medical attention.

In conclusion, while some visual disturbances can be expected after retinal laser treatment, the changes you are experiencing warrant further investigation. Do not hesitate to reach out to an eye care provider for a comprehensive evaluation to ensure that your retina is healing properly and to rule out any potential complications. Your vision is precious, and timely intervention can make a significant difference in your overall outcome.
