Postoperative dizziness
Hello, Dr.
Hsu.
I would like to ask about post-operative issues related to varicocele surgery! About a week after the laparoscopic surgery, I started experiencing scrotal swelling, with both epididymides swollen and the spermatic cord more swollen than before the surgery.
Is this a normal process? It has been about three weeks post-surgery, and the swelling has decreased, but I am troubled by the discomfort caused by the vibrations while riding a motorcycle (especially when hitting a pothole).
I am curious if the swelling contains water or blood.
Another strange phenomenon is that a few days after the surgery, sitting for a long time makes me feel dizzy for a while, but standing does not cause dizziness.
If I engage in DIY activities or sexual intercourse, the dizziness worsens.
I have measured my blood pressure during these dizzy spells, and it has been normal.
I do not have issues with blood sugar or Meniere's disease or any other dizziness problems.
Before the surgery, I noticed that when I experienced discomfort from the varicocele, my left leg would often feel cold and hot due to poor circulation, along with left flank pain.
Fortunately, these symptoms have alleviated and disappeared after the surgery! I wonder if the dizziness is related to the establishment of collateral venous circulation not yet being formed? I would appreciate your advice and suggestions, thank you.

Dr. Xu Weikai reply Urology
Hello, initially, due to the ligation of blood vessels, there may be localized swelling of the testicles.
As for the dizziness, there could be many underlying causes, so it is advisable to seek medical evaluation.
If there are any unclear points, feel free to ask again for further discussion and research.

Post-surgery dizziness and discomfort after varicocele surgery can be concerning, and it's important to understand the potential causes and implications of these symptoms. After undergoing laparoscopic varicocele surgery, it is not uncommon for patients to experience a range of symptoms as their bodies heal.
Firstly, regarding the swelling in the scrotum and epididymis, it is typical to see some degree of edema (swelling) post-surgery. This can be due to the body's inflammatory response to surgery, which can lead to fluid accumulation. The fluid can be serous (clear) or, in some cases, may contain blood if there was any bleeding during the procedure. The swelling should gradually decrease over the weeks following the surgery, and it appears that you are already noticing some improvement at three weeks post-op. However, if the swelling persists or worsens, it would be wise to consult your surgeon for an evaluation.
The discomfort you experience while riding a motorcycle, especially when hitting bumps, could be attributed to the healing tissues and the sensitivity of the surgical site. The scrotum and surrounding areas can be particularly sensitive after surgery, and activities that involve jarring movements can exacerbate discomfort. It is advisable to avoid such activities until you feel more comfortable and have received clearance from your healthcare provider.
Now, concerning the dizziness you experience after sitting for prolonged periods, this symptom can be multifactorial. One possibility is that it could be related to postural changes. After surgery, especially if you have been less active, your body may take time to readjust to changes in blood flow and circulation. When you sit for a long time and then stand up quickly, you might experience orthostatic hypotension, which is a drop in blood pressure upon standing, leading to dizziness. However, since you mentioned that your blood pressure readings are normal, this may not be the primary cause.
Another consideration is the possibility of a transient imbalance in your body's fluid distribution or blood circulation as it adjusts post-surgery. The fact that you experienced cold and hot sensations in your left leg prior to surgery suggests that there may have been some underlying venous insufficiency or circulation issues that could still be in the process of normalizing. The surgery may have improved the venous return from the affected area, but your body may still be adapting to these changes.
It is also worth noting that psychological factors, such as anxiety about the surgery and recovery, can contribute to feelings of dizziness or lightheadedness. If you find that your dizziness persists or worsens, or if you experience additional symptoms such as fainting, severe headaches, or visual disturbances, it is crucial to seek medical attention promptly.
In summary, while some degree of swelling and discomfort is expected after varicocele surgery, persistent or worsening symptoms should be evaluated by your surgeon. Dizziness can be related to various factors, including postural changes, fluid redistribution, or even anxiety. Monitoring your symptoms and maintaining open communication with your healthcare provider will be essential in ensuring a smooth recovery. If you have any further concerns or if your symptoms do not improve, do not hesitate to reach out for a follow-up consultation.
