Red spots on the glans?
Hello, Doctor.
This week, I noticed many red spots on my glans, and some of them have developed into sores that resemble mouth ulcers, which are very painful when touched.
My foreskin is relatively long and can completely cover the glans, but it can be retracted to expose the entire glans, and I do clean it thoroughly while showering.
Could this be balanitis? It has been several days without improvement; although some of the ulcerated areas have healed, the red spots remain, and some still appear swollen.
Even though I am married, I still use condoms during sexual intercourse.
If it is inconvenient to see a doctor, can I buy medication from a pharmacy to use? Being abroad makes it really troublesome to seek medical care.
Thank you, Dr.
Hsu.

More Info
Red spots on the glans, especially when accompanied by painful ulcers, can be concerning and may indicate a variety of conditions. Based on your description, it is possible that you are experiencing a form of balanitis, which is inflammation of the glans (the head of the penis). This condition can be caused by several factors, including infections (bacterial, fungal, or viral), irritants (such as soaps or lotions), or underlying skin conditions (like psoriasis or eczema).
1. Possible Causes:
- Balanitis: This is the most likely cause given your symptoms. It can occur due to poor hygiene, especially if the area is not cleaned regularly, or if there is an overgrowth of yeast or bacteria.
- Sexually Transmitted Infections (STIs): Certain STIs, such as herpes or syphilis, can present with red spots or ulcers on the glans.
- Allergic Reactions: Sometimes, an allergic reaction to latex (from condoms) or other products can cause irritation and redness.
- Dermatological Conditions: Conditions like lichen planus or psoriasis can also manifest as red spots or lesions on the glans.
2. Symptoms: The presence of painful ulcers and swelling is concerning and suggests that the condition may not resolve on its own. If the red spots are accompanied by discharge, fever, or systemic symptoms, it is essential to seek medical attention.
3. Treatment Options:
- Topical Treatments: Over-the-counter antifungal creams (like clotrimazole) may help if a yeast infection is suspected. If it is an allergic reaction, a mild topical steroid cream may reduce inflammation.
- Hygiene: Maintaining good hygiene is crucial. Gently clean the area with mild soap and water, and ensure it is thoroughly dried afterward.
- Avoid Irritants: If you suspect that soaps, lotions, or condoms are causing irritation, consider avoiding these products until the condition improves.
4. Self-Medication: While it may be tempting to self-medicate, it is essential to be cautious. If you are considering purchasing over-the-counter medications, ensure they are appropriate for your symptoms. However, if symptoms persist or worsen, it is crucial to seek medical attention, even if it is inconvenient.
5. When to Seek Medical Help: If the red spots do not improve with home treatment, if you develop fever, or if you notice any unusual discharge, it is vital to consult a healthcare provider. Persistent symptoms could indicate a more serious condition that requires prescription medication or further evaluation.
6. Prevention: Once the current issue is resolved, consider preventive measures such as:
- Regular cleaning of the area.
- Using water-based lubricants if you experience irritation with condoms.
- Avoiding tight clothing that may cause friction.
In summary, while it is possible to manage mild cases of balanitis or irritation with over-the-counter treatments, persistent or severe symptoms warrant professional medical evaluation.
