Increase fertility

Hello, my husband and I only have the opportunity to have intercourse on weekends, but not every week.
If we want to increase our chances of conception, does my husband need to ejaculate a few days before intercourse to release some sperm? I’ve heard that if there hasn’t been intercourse for a while, he should ejaculate first to replenish and accumulate new sperm, which supposedly increases the chances of conception.
Is this true? We are planning to have intercourse around next Friday, which will be the 15th day of my menstrual cycle (28-day cycle).
Should we do this? Thank you.

Dr. Lin Wenbin reply Obstetrics and Gynecology

Although the quality of ejaculation may change after a prolonged period of abstinence, individual differences are significant.
Some individuals may benefit from ejaculating a few days prior to improve the motility of subsequent sperm.
However, this is not the case for everyone.
The best approach is not to limit sexual activity to just holidays.
The key is for couples to follow their physician's guidance and engage in sexual intercourse as needed.
This way, the chances of conception can be enhanced.

More Info

To enhance fertility and improve the chances of conception, it is essential to consider both timing and sperm health. Based on your situation, where you and your husband can only engage in sexual intercourse on weekends, there are several factors to consider regarding sperm health and timing for conception.

Understanding Sperm Health
Sperm health is crucial for successful conception. Factors such as sperm motility, morphology, and concentration play significant roles. From the previous analyses, it appears that your husband has some challenges with sperm motility, which can affect the likelihood of natural conception. Here are some general tips to improve sperm health:
1. Dietary Changes: A balanced diet rich in antioxidants can help improve sperm quality. Foods high in zinc (like nuts, seeds, and whole grains), vitamin C (fruits and vegetables), and omega-3 fatty acids (fish, flaxseeds) are beneficial. Supplements like Coenzyme Q10 and L-carnitine may also support sperm health.

2. Lifestyle Modifications: Encourage your husband to avoid smoking, limit alcohol consumption, and maintain a healthy weight. Regular exercise can improve overall health and potentially enhance sperm quality.

3. Avoiding Heat: Sperm production can be adversely affected by heat. Encourage your husband to avoid hot baths, saunas, and tight clothing that may increase scrotal temperature.

4. Stress Management: High stress levels can negatively impact hormone levels and sperm production. Techniques such as yoga, meditation, or even regular physical activity can help manage stress.

Timing for Conception
Regarding the timing of intercourse, it is essential to align it with the woman's ovulation cycle. Since you mentioned that your menstrual cycle is approximately 28 days, ovulation typically occurs around day 14. Engaging in intercourse during the fertile window (the days leading up to and including ovulation) significantly increases the chances of conception.

1. Frequency of Intercourse: While it is a common belief that abstaining from ejaculation for a few days can lead to a higher concentration of sperm, it is generally recommended to have regular intercourse (every 2-3 days) during the fertile window. This practice helps ensure that sperm are present in the reproductive tract when ovulation occurs.

2. Pre-Ejaculation: There is no need for your husband to "clear out" sperm before the fertile window. In fact, regular ejaculation can help maintain sperm health. Sperm can live in the female reproductive tract for up to five days, so having intercourse a few days before ovulation can still result in conception.

3. Monitoring Ovulation: Using ovulation predictor kits can help identify the most fertile days. This way, you can plan intercourse more effectively around your ovulation cycle.

Conclusion
In summary, while it is important to consider sperm health and timing, there is no need for your husband to ejaculate before your planned intercourse. Instead, focus on maintaining a healthy lifestyle, improving sperm quality through diet and exercise, and timing intercourse around your ovulation. If you continue to face challenges in conceiving, it may be beneficial to consult a fertility specialist for further evaluation and personalized advice. Remember, patience is key in this journey, and maintaining open communication with your partner can help alleviate stress during this time.
