Hair loss issues
Hello, I am 21 years old, and I have been experiencing increasingly severe hair loss over the past six months to a year.
Today, I visited a dermatologist who diagnosed me with early-stage androgenetic alopecia and recommended medication along with a hair growth solution.
The medication prescribed is Propecia (finasteride) 5mg, to be taken as 0.25 tablets for 7 days, once daily.
Could you please tell me what side effects I might expect? Also, will I need to take this medication long-term for it to be effective? Thank you.
Xu, 20~29 year old female. Ask Date: 2013/03/22

Hello: Please refer to Dr.
Wang Hsiu-Han's blog at http://tw.myblog.yahoo.com/skindr-wang/article?mid=1625&prev=-1&next=1624.
The main side effects to be aware of are: 1.
This medication is metabolized by the liver, so it should be used with caution in individuals with liver dysfunction.
2.
This medication is classified as a "5-alpha-reductase type 2 inhibitor," which can inhibit the conversion of testosterone to dihydrotestosterone (DHT).
However, this medication is not classified as an "antiandrogen." Only a small percentage of patients (less than 2%) may experience side effects such as decreased libido, erectile dysfunction, and ejaculation disorders, but it typically does not affect male sexual function.
Furthermore, these side effects are reversible and will resolve after discontinuation of the medication.
This medication needs to be taken long-term to be effective.
Thank you!

More Info
Male pattern baldness, also known as androgenetic alopecia, is a common condition that affects many men, often beginning in their late teens to early twenties. It is characterized by a progressive thinning of hair on the scalp, typically starting at the temples and crown. The condition is primarily driven by genetic factors and the influence of androgens, particularly dihydrotestosterone (DHT), which is a derivative of testosterone.
In your case, the dermatologist has prescribed Propecia (finasteride) at a dosage of 5 mg, which is a common treatment for male pattern baldness. Finasteride works by inhibiting the enzyme 5-alpha reductase, which converts testosterone to DHT. By lowering DHT levels, finasteride can help to slow hair loss and even promote regrowth in some individuals.
Potential Side Effects of Finasteride
While finasteride can be effective, it is important to be aware of potential side effects. Some of the most commonly reported side effects include:
1. Sexual Dysfunction: This can include decreased libido, erectile dysfunction, and ejaculation disorders. These side effects are generally reversible upon discontinuation of the medication, but they can be distressing for some patients.
2. Gynecomastia: Some men may experience breast tenderness or enlargement, which can be uncomfortable and concerning.
3. Mood Changes: There have been reports of mood swings, depression, and anxiety in some individuals taking finasteride. It is important to monitor your mental health while on this medication.
4. Allergic Reactions: Though rare, some individuals may experience allergic reactions, including rash, itching, or swelling.
5. Other Effects: Some users report changes in liver function tests, so regular monitoring may be advised.
Duration of Treatment
Regarding the duration of treatment, finasteride is typically recommended for long-term use to maintain its benefits. Many users begin to see results within three to six months, but it may take up to a year to see the full effects. If you stop taking the medication, any hair regrowth may be lost, and hair loss will likely resume.
Additional Treatment Options
In addition to finasteride, your dermatologist may recommend topical minoxidil (Rogaine), which is another effective treatment for hair loss. Minoxidil works by stimulating hair follicles and increasing blood flow to the scalp. It is often used in conjunction with finasteride for enhanced results.
Lifestyle Considerations
While medications can be effective, lifestyle factors also play a role in hair health. Maintaining a balanced diet rich in vitamins and minerals, particularly those known to support hair health (like biotin, zinc, and vitamins A, C, D, and E), can be beneficial. Additionally, managing stress through exercise, meditation, or other relaxation techniques may help mitigate hair loss.
Conclusion
In summary, finasteride is a well-established treatment for male pattern baldness, but it is essential to be aware of potential side effects and the need for long-term commitment to see results. Regular follow-ups with your dermatologist will help monitor your progress and manage any side effects that may arise. If you experience any concerning symptoms, do not hesitate to reach out to your healthcare provider for guidance.
