Somnolence
Hello Dr.
Tseng: I would like to ask about my condition.
From May 11 to May 13, I experienced prolonged drowsiness and fatigue.
The week prior, I had persistent swelling in my legs and elevated uric acid levels, for which I have been taking medication to manage.
I have a history of hepatitis and have been very busy with work over the past month, leading to insufficient rest.
I am unsure of the cause of my drowsiness, but there are several possibilities.
My father passed away two years ago due to diabetes, and my current lab values are all normal.
Thank you for your response.

1.
There are many causes for drowsiness and lethargy, which can be psychological or physiological.
Physiological causes include medications, infections, organ dysfunction (heart, lungs, kidneys, liver, endocrine), electrolyte imbalances, and abnormalities in blood glucose levels, among other common issues.
Based on your description (from May 11 to May 13), if the symptoms lasted only two to three days and daily activities were not significantly affected, you can monitor the situation for a few days.
Otherwise, you should seek medical evaluation at a nearby internal medicine clinic.
2.
Swelling in the feet is not necessarily caused by high uric acid levels (typical gout symptoms include sudden redness, swelling, pain, and heat in the joints, with symptoms resolving within a week).
Since the specific location of the swelling was not detailed, it is not possible to confirm whether it is gouty arthritis.
Additionally, it is worth mentioning that hyperuricemia, if not accompanied by more than three episodes of gouty arthritis, the formation of tophi, or associated cardiovascular or cerebrovascular diseases, can generally be managed through dietary control and exercise without medication.
It is important to avoid taking medications indiscriminately or relying on herbal remedies.

Dear Inquirer,
Thank you for your question regarding your persistent sleepiness and the various symptoms you have been experiencing. It seems that you are dealing with a complex situation that may involve multiple factors contributing to your current state of fatigue and malaise. Let's break down the potential causes and concerns based on the information you've provided.
1. Medical Conditions: You mentioned having hepatitis and elevated uric acid levels. Both of these conditions can significantly impact your overall health and energy levels. Hepatitis, depending on its severity and the extent of liver function impairment, can lead to fatigue, weakness, and a general sense of malaise. Elevated uric acid levels can lead to gout, which may cause joint pain and swelling, further contributing to your discomfort and fatigue. It is crucial to maintain regular follow-ups with your healthcare provider to monitor these conditions and adjust your treatment plan as necessary.
2. Sleep Quality and Quantity: You indicated that you have been working excessively and not getting enough rest. Chronic sleep deprivation can lead to a host of issues, including persistent sleepiness, difficulty concentrating, and mood disturbances. The body requires adequate sleep to recover and rejuvenate, and when this is compromised, it can manifest as overwhelming fatigue. It is essential to prioritize sleep hygiene, which includes establishing a regular sleep schedule, creating a comfortable sleep environment, and avoiding stimulants like caffeine close to bedtime.
3. Psychological Factors: The loss of your father due to diabetes may also be weighing heavily on your mind. Grief can manifest in various ways, including fatigue and sleep disturbances. If you find that your emotional state is affecting your daily functioning, it may be beneficial to seek support from a mental health professional. They can help you process your feelings and develop coping strategies.
4. Lifestyle Factors: You mentioned a busy work schedule, which can lead to stress and burnout. Stress can significantly impact your physical and mental health, leading to fatigue and sleep issues. It is important to find a balance between work and personal time. Incorporating relaxation techniques such as mindfulness, meditation, or gentle exercise can help alleviate stress and improve your overall well-being.
5. Potential Sleep Disorders: If your sleepiness persists despite making lifestyle changes, it may be worth exploring the possibility of a sleep disorder, such as sleep apnea or restless leg syndrome. These conditions can disrupt your sleep quality and lead to excessive daytime sleepiness. A sleep study may be recommended by your healthcare provider to assess your sleep patterns and identify any underlying issues.
In conclusion, your persistent sleepiness could be attributed to a combination of medical conditions, lifestyle factors, psychological stressors, and potential sleep disorders. It is essential to consult with your healthcare provider to discuss your symptoms in detail. They may recommend further testing or referrals to specialists, such as a sleep medicine expert or a mental health professional, to address your concerns comprehensively.
Remember, taking care of your physical and mental health is paramount. Prioritize rest, seek support when needed, and stay proactive about your health. Wishing you the best in your journey toward improved well-being.
Best regards.
