How to detect leg length discrepancy?

Hello, how can I self-assess whether I have leg length discrepancy? If I do have a leg length discrepancy, what are the methods for correction and treatment? I would appreciate Dr.
Chen taking the time to respond.
Thank you!

Xiao Ma, 40~49 year old female. Ask Date: 2013/05/26

Hello: In response to your inquiry regarding the measurement of leg length discrepancy, there are generally two main categories: manual measurement and imaging measurement.
Manual measurement is convenient and quick but may lead to errors, while imaging measurement is more accurate but requires exposure to some radiation.
Self-assessment by patients is also possible.
Here are three simple self-assessment methods:
1.
Lie flat naturally with both feet together along the midline.
Ask someone to measure the distance between points A and B on the left side (A: anterior superior iliac spine, B: inferior border of the medial malleolus) using a tape measure.
Then measure the distance between the same points on the right side and check if the difference exceeds 0.5 cm (Note: The anterior superior iliac spine is located at the level of the waist where a belt would sit, aligned with the midline of the nipples).
2.
In the same position, with both feet together along the midline, ask someone to lift the subject's toes upward and visually assess whether the lengths of the heels on both sides are equal.
3.
Sit with both knees and feet together at a 90-degree angle.
The subject should observe the length of the thighs (the distance from the hip to the knee) to see if both sides match.
Additionally, in the same position, someone in front of the subject should compare the height of the knees from the ground (the length of the calves) to see if both sides are consistent.
Please note that self-assessment methods may have inaccuracies.
As for the correction of leg length discrepancy, it is essential to identify the underlying cause, which can vary depending on the patient's age, and cannot be generalized.
If there are any concerns, it is advisable to seek further assistance from an orthopedic specialist.

To self-check for leg length discrepancy (LLD), you can follow a simple method at home. Here’s a step-by-step guide:
1. Preparation: Wear comfortable clothing and stand on a flat, hard surface. Ensure you have a measuring tape and a helper if possible.

2. Standing Measurement:
- Stand upright with your feet together and your weight evenly distributed on both legs.

- Ask someone to measure from the anterior superior iliac spine (ASIS) of one hip to the medial malleolus (the bony prominence on the inside of your ankle) of the same leg. This is the most accurate way to measure leg length.

- Repeat the measurement on the other leg.

- Compare the two measurements. A difference of more than 1 cm (0.4 inches) may indicate a significant leg length discrepancy.

3. Supine Measurement:
- Lie flat on your back on a hard surface with your legs extended.

- Measure from the ASIS to the heel of each foot. Again, compare the two measurements.

4. Functional Tests:
- You can also perform a simple functional test. Stand on one leg and see if you can maintain balance. If you find it significantly harder to balance on one leg, it could indicate a discrepancy.

If you suspect you have a leg length discrepancy, it’s important to consult a healthcare professional for a thorough evaluation. They may perform additional tests, such as X-rays or physical examinations, to confirm the diagnosis.

Treatment Options for Leg Length Discrepancy
1. Orthotic Devices:
- If the discrepancy is minor (less than 2 cm), custom orthotics or shoe lifts can help balance the legs. These can be placed inside the shoe of the shorter leg to help equalize the length and improve posture.

2. Physical Therapy:
- A physical therapist can provide exercises to strengthen the muscles around the hips and legs, which can help compensate for the discrepancy. They may also teach you stretching techniques to improve flexibility and reduce discomfort.

3. Surgical Options:
- For more significant discrepancies (greater than 2 cm), surgical options may be considered. These can include:
- Limb Lengthening Surgery: This involves cutting the bone and gradually pulling it apart using a device to allow new bone to form in the gap.

- Bone Shortening Surgery: In cases where one leg is significantly longer, the surgeon may remove a portion of the bone to shorten it.

4. Postural Training:
- Learning proper posture and body mechanics can help alleviate some of the discomfort associated with leg length discrepancies. This includes techniques for walking, standing, and sitting that promote balance and alignment.

5. Regular Monitoring:
- If you have a diagnosed leg length discrepancy, regular follow-ups with a healthcare provider are essential to monitor any changes and adjust treatment as necessary.

Conclusion
Leg length discrepancy can lead to various issues, including pain, discomfort, and postural problems. Self-checking can provide initial insights, but a professional evaluation is crucial for accurate diagnosis and treatment. Depending on the severity of the discrepancy, treatment options range from conservative measures like orthotics and physical therapy to surgical interventions. Always consult with a healthcare professional before starting any treatment plan to ensure it is appropriate for your specific condition.
