Premature Ejaculation and Semen Changes: A Urology Perspective - Urology

Please help me clarify the doubts in my mind, doctor?

Hello, Dr.
Hsu.
About a month and a half ago, after I satisfied my physiological needs, I felt a soreness in my genital area.
Since then, during my recent attempts at self-pleasure, I have been unable to maintain an erection and have ejaculated before achieving full hardness.
Additionally, I have noticed that my heart rate increases significantly during these attempts.
From June until last week, I did not engage in self-pleasure, but today, after a long period without exposure to highly stimulating material and considering my previous experiences, I again ejaculated before achieving full hardness.
My heart rate was still very fast during the act.
Therefore, I would like to ask Dr.
Hsu:
1.
Does this qualify as premature ejaculation or erectile dysfunction, or is it simply due to excessive nervousness? (I experience morning erections almost every day, and their firmness is generally good, lasting over 5 minutes, sometimes even up to half an hour.
However, whenever I think about self-pleasure, my heart races, and I find it difficult to achieve full engorgement.)
2.
During these recent instances of masturbation, I have noticed small, transparent solid pieces in my semen.
After some time (about half an hour), the semen liquefies successfully, and the jelly-like consistency almost disappears, making it difficult to observe.
When I do notice them, their volume is significantly reduced.
Does this indicate any issues with my urinary system? (The color of the semen is milky white.) I would greatly appreciate Dr.
Hsu's insights!

Is it too much anxiety? Hello,
1.
Premature ejaculation requires more clinical data.
It cannot be diagnosed based solely on a few instances.

2.
There may not be any issues; it might be best to observe for a while.
However, if you are still concerned, you can visit a nearby laboratory for a semen analysis.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Answers from the internet should only serve as a reference; if necessary, it is important to seek medical care at a healthcare facility.

This message is from Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.
We care about you.

Understanding premature ejaculation and changes in semen can be complex, especially when considering the psychological and physiological factors involved. Based on your description, let's break down your concerns and provide some insights.

1. Premature Ejaculation vs. Anxiety:
From what you've described, it seems that you are experiencing episodes of ejaculation before you feel fully aroused or ready. This can indeed be classified as premature ejaculation (PE) if it occurs consistently and is beyond your control. However, given that you also mention feeling anxious and having a rapid heartbeat during these episodes, it is possible that anxiety is playing a significant role. Anxiety can lead to heightened arousal responses, which may cause you to ejaculate sooner than you would like. The fact that you experience morning erections and can maintain an erection for a reasonable duration during other times suggests that your erectile function is generally intact.
It’s important to differentiate between situational issues (like anxiety during masturbation) and a more chronic condition (like PE). If this is a recurring issue that affects your sexual satisfaction or causes distress, it may be worth discussing with a healthcare provider who specializes in sexual health.

2. Changes in Semen Consistency:
The presence of small, transparent solid pieces in your semen can be concerning, but it is not uncommon for semen to have variations in consistency. Semen typically consists of sperm and seminal fluid, and its appearance can change based on various factors, including hydration, diet, and frequency of ejaculation. The fact that your semen liquefies after a period is normal; semen can initially appear gel-like and then liquefy to facilitate sperm mobility.

However, if you notice persistent changes in the appearance of your semen, such as unusual color, consistency, or if you experience pain, discomfort, or other symptoms (like changes in urination), it would be advisable to consult a urologist. They can perform a thorough evaluation, including a semen analysis if necessary, to rule out any underlying issues such as infections or other urological conditions.

3. Psychological Factors:
Your experience of rapid heartbeat and anxiety during masturbation suggests that psychological factors may be influencing your sexual function. Stress and anxiety can significantly impact sexual performance and satisfaction. Techniques such as mindfulness, relaxation exercises, or even cognitive behavioral therapy (CBT) can be beneficial in managing anxiety related to sexual performance.

4. General Recommendations:
- Relaxation Techniques: Consider practicing relaxation techniques before engaging in sexual activity or masturbation. Deep breathing, meditation, or progressive muscle relaxation can help reduce anxiety.

- Communication: If you are in a relationship, open communication with your partner about your feelings and experiences can help alleviate some of the pressure you may feel.

- Professional Help: If these issues persist, consider seeking help from a urologist or a therapist specializing in sexual health. They can provide tailored advice and treatment options.

In conclusion, while your experiences may indicate some issues with premature ejaculation and changes in semen consistency, they are not uncommon and can often be managed with appropriate strategies and professional guidance. It’s essential to address both the physical and psychological aspects of sexual health to achieve a satisfying sexual experience.
