Long-acting testosterone preparations
Hello Dr.
Hsu: It can be confirmed that I have a severe deficiency in testosterone, as evidenced by various symptoms such as insomnia, widespread muscle pain, and most seriously, erectile dysfunction with inability to achieve an erection.
There are now long-acting formulations available for patients to address these issues.
Many related articles and promotional materials can be found online, and there are numerous praises in advertisements from urology clinics.
I have heard that the results are quite good, but I am unsure of their credibility.
When I called the clinic for inquiries, I only spoke directly with the doctors, and the prices were exorbitantly high, which is quite puzzling.
I would like to understand if this medication can truly last for three months.
Will I need to continue receiving injections every three months thereafter? If the results are not as claimed, what happens if I stop the treatment? Are there any side effects or long-term consequences to consider? This is a safety concern that should be addressed.
Additionally, if this medication is administered at a public hospital outpatient clinic, will the cost be significantly lower than the outrageous prices at private clinics? I hope you can provide accurate information to help patients receive fair treatment.
It would greatly relieve my concerns.
Thank you very much.

T-therapy: Long-acting male hormone preparations are said to last for 1-3 months.
There are many supplement forms available, including oral, injectable, and topical applications.
However, these medications can have side effects, and some may even increase the risk of cancer.
Therefore, I only recommend their use after confirming a deficiency in hormone levels through blood tests.
Currently, our hospital does not offer male hormone preparations.
You may consult a urologist at a nearby facility.
The pricing at public hospitals is determined by the drug procurement system established by the Ministry of Health and Welfare, rather than being set by the hospitals themselves.
Note: Taipei City Hospital also has a similar procurement mechanism.
If you have further questions, feel free to ask, and we can discuss and research together.

Long-acting testosterone injections, such as testosterone undecanoate or testosterone cypionate, have gained popularity for treating testosterone deficiency, particularly in men experiencing symptoms like fatigue, decreased libido, and mood changes. These injections can provide significant benefits, but they also come with potential concerns that patients should be aware of.
Benefits of Long-Acting Testosterone Injections
1. Sustained Release: One of the primary advantages of long-acting testosterone injections is their ability to maintain stable testosterone levels over an extended period, typically up to three months. This can help alleviate symptoms associated with low testosterone, such as fatigue, muscle weakness, and sexual dysfunction.
2. Convenience: With injections administered every three months, patients may find this regimen more convenient compared to daily or weekly testosterone replacement therapies. This can improve adherence to treatment, as patients do not have to remember to take a pill every day.
3. Improved Quality of Life: Many patients report significant improvements in their quality of life, including enhanced mood, increased energy levels, and improved sexual function. These benefits can lead to better overall well-being and satisfaction.
Concerns and Considerations
1. Side Effects: While testosterone therapy can be beneficial, it is not without risks. Common side effects may include acne, fluid retention, increased red blood cell count (which can lead to blood clots), and potential worsening of sleep apnea. Some patients may also experience mood swings or aggression.
2. Hormonal Imbalance: Discontinuing testosterone therapy suddenly can lead to withdrawal symptoms and a return of low testosterone symptoms. It's essential to discuss with your healthcare provider the best approach to taper off the medication if needed.
3. Monitoring: Regular monitoring of testosterone levels, hematocrit (to check for increased red blood cell count), and prostate-specific antigen (PSA) levels is crucial during treatment. This helps ensure that the therapy is effective and minimizes the risk of adverse effects.
4. Cost Considerations: The cost of long-acting testosterone injections can vary significantly between private clinics and public hospitals. While private clinics may charge higher fees, public hospitals often provide more affordable options. It’s advisable to inquire about pricing and insurance coverage before starting treatment.
5. Long-term Safety: The long-term safety of testosterone therapy is still being studied. While many patients tolerate the treatment well, there are concerns about potential cardiovascular risks, particularly in older men or those with pre-existing heart conditions. It's essential to have a thorough discussion with your healthcare provider about your individual risk factors.
Conclusion
Long-acting testosterone injections can be an effective treatment for testosterone deficiency, offering sustained relief from symptoms and improved quality of life. However, patients should be aware of the potential side effects and the importance of regular monitoring. If you are considering this treatment, it is crucial to have an open dialogue with your healthcare provider to ensure that it aligns with your health needs and to discuss any concerns you may have about costs and long-term safety.
