Postoperative Issues After Hemorrhoid Surgery
Dr.
Ke: I had surgery for internal and external hemorrhoids.
On the tenth day post-surgery, while lying down, my entire buttocks suddenly became wet, and it turned out to be blood.
It lasted only a few seconds.
An hour later, my stool was black at the front, but normal at the back.
What could that be? On the 11th day post-surgery, I noticed that my wound was opening up and had pus.
Now it's the 12th day, and even after applying medication, I still have a few instances of pus discharge daily.
I went back to see my attending physician, and he said this is a normal phenomenon.
Is that really the case? Currently, I have no pain at all, but I do notice a little bit of blood during bowel movements, along with slight discomfort.

Hello: In response to your question, mild bleeding may occur on the tenth day post-surgery due to the absorption of sutures.
As long as there is no continuous heavy bleeding, it should not be a concern.
During the healing process, the surgical wound may produce some discharge, which is a normal phenomenon.
This discharge is often mistaken for pus, but as long as there is no fever or severe pain, it should be fine.
It is recommended to take warm sitz baths to alleviate this phenomenon.

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience various post-operative symptoms, including bleeding and discharge. Your situation, where you noticed a sudden wetness with blood and later observed a change in the wound, raises several important points worth discussing.
Firstly, the sudden bleeding you experienced on the tenth day post-surgery could be attributed to several factors. After hemorrhoid surgery, the surgical site is still healing, and it is possible for small blood vessels to reopen, leading to bleeding. The fact that you mentioned the blood was only present for a few seconds and then subsided may indicate that it was a minor issue, possibly related to the healing process or even a small rupture of a blood vessel. However, the presence of black stool (melena) suggests that there may have been some bleeding higher up in the gastrointestinal tract, which could be a cause for concern. Melena typically indicates that the blood has been digested, which can occur from bleeding in the upper GI tract, but it is essential to correlate this with your overall condition and any other symptoms you may have.
Regarding the discharge you are experiencing, it is not unusual for surgical wounds to ooze some fluid, especially if there is an infection or if the wound is not healing properly. The presence of pus (which you referred to as "化濃") could indicate an infection at the surgical site. While your surgeon may have reassured you that this is a normal phenomenon, it is crucial to monitor the situation closely. If the discharge continues, increases, or is accompanied by other symptoms such as fever, increased pain, or swelling, it would be prudent to follow up with your healthcare provider for further evaluation.
The fact that you are not experiencing significant pain is a positive sign, as severe pain could indicate complications such as an abscess or other issues. However, the slight bleeding during bowel movements is something to keep an eye on. It is common for patients to experience some bleeding after hemorrhoid surgery, especially if they are straining during bowel movements or if the stool is hard. Ensuring that you maintain a soft stool through adequate hydration and dietary fiber can help minimize this risk.
In summary, while some bleeding and discharge can be normal after hemorrhoid surgery, it is essential to remain vigilant. If you notice any changes in your symptoms, such as increased bleeding, worsening discharge, or new symptoms, do not hesitate to seek medical attention. Your healthcare provider can assess your situation more thoroughly and determine if any additional treatment or intervention is necessary. It is always better to err on the side of caution when it comes to post-operative care.
