Missed menstruation?

Hello Dr.
Dong, I would like to ask: My menstrual cycle has always been regular, ranging from 25 to 30 days.
However, it has become irregular over the past year.
My last period was around March 24, and it still hasn't arrived.
During this time, I visited a gynecologist in May, and after an ultrasound, the doctor said everything was fine and prescribed me a three-day course of medication to induce menstruation, stating that my period would come within a week.
It did not come, so I consulted a traditional Chinese medicine practitioner, thinking of trying herbal medicine, but after taking it two or three times, my period still hasn't arrived.
I would like to know if I need to undergo any tests in this situation, or if it's just due to my age and I should let it be.
I appreciate your time in responding, as I feel anxious about it—whether it comes or not, it's troubling.
Thank you! Grateful~

Xiao Guai, 40~49 year old female. Ask Date: 2013/08/20

Irregular periods can be a source of concern for many women, especially when they have previously experienced regular menstrual cycles. Your situation, where your menstrual cycle has shifted from a consistent 25-30 days to a more unpredictable pattern, is not uncommon, particularly as women approach different life stages, including perimenopause.
Firstly, it’s important to understand that menstrual irregularities can be caused by a variety of factors. These can include hormonal imbalances, stress, significant weight changes, excessive exercise, or underlying health conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders. Given that you have already consulted with a gynecologist and had an ultrasound that showed no issues, it seems that some of the more serious conditions have been ruled out. However, the persistence of your irregularity warrants further exploration.

The fact that you were prescribed medication to induce your period indicates that your doctor is considering hormonal factors. If the medication did not work, it may suggest that there are other underlying issues at play. Traditional Chinese medicine (TCM) can be beneficial for some women in regulating their menstrual cycles, but it may take time to see results. If you have only taken the herbal remedies a few times, it might be worth continuing for a longer period, as TCM often requires consistent treatment to be effective.

In terms of further evaluations, if your irregular periods continue, it may be advisable to undergo additional tests. These could include blood tests to check hormone levels, such as estrogen, progesterone, thyroid hormones, and possibly prolactin. A complete blood count (CBC) and other metabolic panels can also help rule out other health issues. If you have not yet had a pelvic exam or Pap smear recently, it might be worth discussing with your healthcare provider, as these can provide additional insights into your reproductive health.

Regarding your concern about age and whether to "let nature take its course," it’s important to consider your overall health and lifestyle. If you are experiencing significant stress, lifestyle changes, or other symptoms (like weight gain, acne, or excessive hair growth), these could indicate a hormonal imbalance that may require intervention.
If you find that your irregular periods are accompanied by other symptoms such as severe pain, heavy bleeding, or significant mood changes, it is crucial to seek medical advice sooner rather than later. These could be signs of conditions that need treatment.

In summary, while some irregularity can be normal, especially as you age, it is essential to monitor your symptoms and stay in communication with your healthcare provider. If your periods do not return to a more regular pattern, or if you experience any concerning symptoms, further evaluation may be necessary. It’s always better to err on the side of caution when it comes to your health.
