Evening Itch: Possible Causes and Solutions - Dermatology

Hello, doctor.
I apologize for bothering you, but I would like to ask a question.
About ten days ago, I started experiencing itching every evening around seven or eight o'clock.
It initially started on my feet, but now it has spread to my entire body.
The more I scratch, the larger the welts become (they look similar to mosquito bites).
However, my daily routine has been normal, and I haven't eaten anything unusual.
I did some research online, and it seems to resemble urticaria (hives), but does this condition typically have a fixed time of onset? It started as localized itching, but now it itches all over my body.

Ya Yuan, 30~39 year old female. Ask Date: 2013/08/22

Dr. Li Yufen reply Dermatology

Hello: General urticaria does not have a fixed time of occurrence.
It is usually triggered by consuming allergenic medications or foods, or by exposure to environmental allergens.
You can pay attention to whether there is a common factor before each outbreak, such as exposure to the same substances or being in the same environment.
Thank you!

Evening itchiness can be a perplexing and uncomfortable experience, especially when it seems to follow a predictable pattern. Based on your description, it appears that you are experiencing a condition that may be similar to urticaria, commonly known as hives. This condition can indeed manifest at specific times, often in response to various triggers.
Hives are characterized by raised, itchy welts on the skin that can vary in size and can appear anywhere on the body. They can be caused by a variety of factors, including allergic reactions, stress, temperature changes, or even certain medications. In your case, the fact that the itching intensifies in the evening could suggest a few possibilities:
1. Environmental Factors: The evening hours may bring about changes in your environment, such as increased humidity or exposure to allergens like dust mites or pet dander. If you have recently changed your living environment or if there are new allergens present, this could be a contributing factor.

2. Circadian Rhythms: The human body operates on a circadian rhythm, which can influence various physiological processes, including skin reactions. Some individuals may find that their skin becomes more reactive in the evening due to hormonal fluctuations or changes in body temperature.

3. Dietary Triggers: While you mentioned that your diet has been normal, it’s worth considering if there are any foods you consume regularly in the evening that could be contributing to your symptoms. Common culprits include alcohol, spicy foods, or foods high in histamines.

4. Stress and Anxiety: Emotional stress can exacerbate skin conditions, including hives. If you find that your evenings are particularly stressful, this could be a factor in your symptoms.

5. Infections or Skin Conditions: Certain infections or skin conditions can also cause itching. If the itching is accompanied by other symptoms, such as fever, swelling, or a rash, it may be worth consulting a healthcare professional for further evaluation.

To manage your symptoms, consider the following strategies:
- Avoid Scratching: While it may be tempting, scratching can worsen the irritation and lead to further inflammation or infection. Instead, try applying a cool compress to the affected areas to soothe the itch.

- Over-the-Counter Antihistamines: Medications such as diphenhydramine (Benadryl) or cetirizine (Zyrtec) can help alleviate itching and reduce the severity of hives. However, be mindful of potential drowsiness with some antihistamines.

- Moisturizers: Keeping your skin well-hydrated can help reduce itchiness. Look for fragrance-free moisturizers that are designed for sensitive skin.

- Identify Triggers: Keeping a diary of your symptoms, including when they occur and any potential triggers, can help you identify patterns and avoid specific irritants.

- Consult a Dermatologist: If your symptoms persist or worsen, it’s advisable to seek professional medical advice. A dermatologist can help diagnose the underlying cause of your itching and recommend appropriate treatments.

In summary, while evening itchiness can be distressing, understanding potential triggers and implementing effective management strategies can help alleviate your symptoms. If the condition continues to disrupt your daily life, do not hesitate to reach out to a healthcare provider for further evaluation and treatment options.
