Postoperative condition after fistula surgery?
Hello Dr.
Ke: I underwent fistula surgery on August 23.
The surgeon mentioned that the wound is shallow and that the internal opening was successfully excised.
I returned home for recovery the same day and did not require hospitalization.
I started my first sitz bath before bed.
I have a question: the next day, I noticed a small amount of light red blood on the sanitary pad I placed in my underwear (considering there might be discharge and I didn't want to change underwear frequently).
Is this a normal occurrence? Additionally, are there any specific dietary or lifestyle precautions I should be aware of after the surgery? Thank you for your guidance.

Hello: In response to your question, it is normal to have discharge and slight bleeding from the wound for up to a month after anal fistula surgery until the wound is fully healed.
It is recommended to take warm sitz baths frequently and to avoid spicy and irritating foods in your diet.
Additionally, it is important to increase your intake of dietary fiber to prevent constipation.

Post-surgery care after fistula surgery is crucial for a smooth recovery and to minimize complications. It’s great to hear that you had your surgery on August 23 and that your surgeon has indicated that the wound is shallow and the internal opening has been successfully addressed.
Regarding your concern about the slight blood on the sanitary pad the day after surgery, it is not uncommon to experience some light bleeding or discharge following such a procedure. This can be due to the healing process as the body begins to repair the surgical site. As long as the bleeding is minimal and not accompanied by severe pain, fever, or other concerning symptoms, it is generally considered normal. However, if you notice an increase in bleeding, or if the blood becomes bright red and heavy, it would be wise to contact your healthcare provider for further evaluation.
In terms of post-operative care, here are several recommendations to ensure a smooth recovery:
1. Wound Care: Keep the surgical area clean and dry. Follow your surgeon's instructions regarding how to clean the area and when to change dressings. If you notice any signs of infection, such as increased redness, swelling, warmth, or discharge that is foul-smelling, contact your doctor immediately.
2. Sitz Baths: You mentioned starting sitz baths, which can be very beneficial. They help keep the area clean and can provide relief from discomfort. Typically, sit in warm water for about 15-20 minutes several times a day, especially after bowel movements.
3. Dietary Considerations: After fistula surgery, it’s important to maintain a diet that promotes healing. Focus on high-fiber foods to prevent constipation, which can put strain on the surgical site. Foods like fruits, vegetables, whole grains, and plenty of fluids can help. Avoid spicy foods and caffeine, as they may irritate the digestive system.
4. Activity Level: Rest is essential in the initial days following surgery. Avoid heavy lifting or strenuous activities that could strain the surgical area. Gradually increase your activity level as you feel comfortable, but listen to your body.
5. Pain Management: If you experience discomfort, over-the-counter pain relievers like acetaminophen or ibuprofen can be helpful. However, consult your doctor before taking any medication, especially if you have other health conditions or are taking other medications.
6. Follow-Up Appointments: Ensure you attend all scheduled follow-up appointments with your surgeon. These visits are important for monitoring your recovery and addressing any concerns that may arise.
7. Signs to Watch For: Be vigilant for any unusual symptoms such as increased pain, swelling, fever, or changes in bowel habits. If you experience any of these, reach out to your healthcare provider.
8. Emotional Well-being: Surgery can be a stressful experience, and it’s important to take care of your mental health as well. Don’t hesitate to reach out to friends, family, or professionals if you feel overwhelmed.
In summary, while some light bleeding post-surgery can be normal, maintaining proper wound care, following dietary recommendations, and monitoring your symptoms are key to a successful recovery. If you have any further questions or concerns, don’t hesitate to reach out to your healthcare provider for personalized advice. Wishing you a smooth and speedy recovery!
