Postoperative care after varicose vein surgery?
Hello doctor, my husband underwent microsurgery for varicose veins the day before yesterday.
Currently, the wound doesn't hurt too much, but his testicles are swollen and there seems to be some bruising.
Is this a normal phenomenon? Should we apply ice or heat to the testicles? Can he take some herbal medicine to reduce the swelling? Otherwise, he has to walk with his legs spread apart.

Hello, first of all, the surgery involves ligating the veins responsible for the blood flow from the testicles, which can cause resistance to the returning blood.
Immediately after the surgery, it is indeed common for the testicles to feel swollen; however, the degree of swelling can vary based on individual constitution and the surgical process.
Generally, after the surgery, it is recommended to apply ice for three days, followed by heat for the next three days.
If heat application is uncomfortable, it is acceptable to continue with ice for a few more days.
Note: On average, it takes about six months for varicocele to improve post-surgery.
As for herbal medicine, I am not familiar with that.

After undergoing varicocele surgery, it is not uncommon for patients to experience some swelling and bruising in the scrotal area. This is generally a normal part of the healing process. The surgery involves manipulating the veins in the scrotum, which can lead to localized trauma and subsequent inflammation. Here’s a detailed explanation of what your husband might be experiencing and some recommendations for managing his symptoms.
Understanding Swelling and Bruising
1. Normal Post-Operative Symptoms: After varicocele surgery, patients often report swelling and bruising around the surgical site. This is due to the surgical manipulation of tissues and blood vessels. The body’s inflammatory response to surgery can cause fluid accumulation, leading to swelling. Bruising occurs when small blood vessels are damaged during the procedure, causing blood to leak into surrounding tissues.
2. Duration of Symptoms: Typically, swelling and bruising can last for several days to a couple of weeks. The severity and duration can vary depending on the individual’s healing process, the extent of the surgery, and how well the patient follows post-operative care instructions.
Management of Symptoms
1. Ice Therapy: Applying ice packs to the affected area can help reduce swelling and numb the pain. It is generally recommended to apply ice for 15-20 minutes every hour as needed during the first 48 hours post-surgery. Make sure to wrap the ice pack in a cloth to avoid direct contact with the skin, which can cause frostbite.
2. Heat Therapy: After the initial 48 hours, some patients find relief from swelling and discomfort by using heat. A warm compress can help improve blood flow and promote healing. However, it’s essential to monitor how your husband feels with heat application, as some may prefer cold therapy throughout the recovery period.
3. Positioning: Encourage your husband to rest with his legs elevated when possible. This can help reduce swelling by promoting venous return and minimizing fluid accumulation in the scrotal area.
4. Pain Management: Over-the-counter pain relievers such as acetaminophen or ibuprofen can help manage discomfort. However, he should consult with his doctor before taking any medication, especially if he is considering herbal remedies or traditional Chinese medicine.
5. Activity Level: It’s crucial for your husband to avoid strenuous activities, heavy lifting, or any activities that could strain the abdominal area for at least a week post-surgery. Gentle walking is encouraged to promote circulation but should be done at a comfortable pace.
Herbal Remedies
While some people may consider using herbal remedies to reduce swelling, it’s essential to consult with a healthcare provider before introducing any new treatments. Some herbs may interact with medications or have side effects that could complicate recovery. If your husband is interested in using herbal remedies, he should discuss this with his surgeon or a qualified herbalist who understands his medical history.
When to Seek Medical Attention
While swelling and bruising are typically normal, there are certain signs that should prompt a call to the healthcare provider:
- Increased Swelling or Pain: If the swelling worsens or if there is significant pain that is not relieved by medication.
- Signs of Infection: Redness, warmth, or discharge from the surgical site, or fever.
- Changes in Urination: Difficulty urinating or blood in the urine.
In summary, swelling and bruising after varicocele surgery are common and usually resolve with proper care. Ice therapy, rest, and monitoring symptoms are key to a smooth recovery. If there are any concerns or unusual symptoms, it’s always best to consult with the surgeon for personalized advice.
