Post tympanoplasty surgery?
Doctor, I would like to ask, I recently had tympanic membrane repair surgery about a week ago.
Today, I had the stitches removed and the packing taken out of my ear canal, but it feels like the packing is still there, similar to tinnitus.
My hearing was not significantly different before the surgery.
Could this be because the tympanic membrane has not fully healed yet? Is this normal? How long might it take for things to return to normal? Thank you.
Xiao Yu, 20~29 year old female. Ask Date: 2013/12/28
Dr. Zheng Jueyi reply Otolaryngology
Hi Xiaoyu: Hello, if a tympanoplasty is performed under general anesthesia, special dressings are placed both inside and outside the newly repaired tympanic membrane for stabilization.
It is common to experience tinnitus or a sensation of fullness in the ear immediately after the surgery.
This phenomenon should improve as the dressings gradually absorb, typically within one to two months.
It is recommended to continue follow-up visits for monitoring.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan General Hospital, Ministry of Health and Welfare.

After undergoing tympanoplasty, it is not uncommon for patients to experience symptoms such as tinnitus (ear ringing) and fluctuating hearing. Your concerns about the persistence of these symptoms one week post-surgery are valid and warrant a detailed explanation.
Firstly, it's important to understand that tympanoplasty is a surgical procedure aimed at repairing a perforated eardrum (tympanic membrane) and restoring hearing. While the surgery is generally successful, the recovery process can vary significantly among individuals. The symptoms you are experiencing, including the sensation of ear ringing and the feeling that your ear is still blocked, can be attributed to several factors related to the healing process.
1. Healing Process: After tympanoplasty, the tympanic membrane needs time to heal. The surgical site may still be inflamed, and the tissues are in the process of regenerating. This healing phase can lead to temporary changes in hearing and the perception of sounds, including tinnitus. It is not unusual for patients to report that their hearing fluctuates during this period, as the ear adjusts to the changes.
2. Eustachian Tube Function: The Eustachian tube, which connects the middle ear to the back of the throat, plays a crucial role in equalizing pressure in the ear. If the Eustachian tube is not functioning optimally post-surgery, you may experience a sensation of fullness or pressure in the ear, which can contribute to the feeling of ear ringing. This dysfunction can also affect hearing clarity. It is essential to monitor how your Eustachian tube is functioning, as it can impact your overall recovery.
3. Tinnitus: Tinnitus can occur for various reasons, including changes in the inner ear or auditory pathways following surgery. It may be a result of the surgical trauma, inflammation, or even the healing process itself. While it can be distressing, many patients find that their tinnitus diminishes as the ear heals. The duration of tinnitus post-surgery varies; some may notice improvement within weeks, while others may take months to see significant changes.
4. Post-Operative Care: Following your surgeon's post-operative care instructions is crucial for a smooth recovery. Avoid inserting anything into the ear, including cotton swabs, as this can disrupt the healing process. If you experience increased pain, discharge, or any other concerning symptoms, it is essential to contact your healthcare provider promptly.
5. Timeline for Recovery: While every patient's recovery timeline is different, many individuals begin to notice improvements in their symptoms within a few weeks post-surgery. However, complete healing of the tympanic membrane and resolution of symptoms can take several months. It is essential to maintain regular follow-up appointments with your ENT specialist to monitor your progress and address any ongoing concerns.
In summary, your experience of ear ringing and fluctuating hearing after tympanoplasty is not uncommon and can be attributed to the healing process, Eustachian tube function, and the nature of tinnitus. While it may take some time for your symptoms to resolve, maintaining open communication with your healthcare provider and adhering to post-operative care will support your recovery. If symptoms persist or worsen, do not hesitate to seek further evaluation and management from your ENT specialist.
