Is taking medication this way good for the body and kidneys?

Due to a severe brain injury from a car accident last April, my Western medicine doctor prescribed me to take Choline and Vitamin B, specifically one Vitamin B in the morning and one Choline after each meal.
However, I later switched to acupuncture treatment with a Traditional Chinese Medicine (TCM) practitioner, who, after taking my pulse, prescribed Huanglian Jiedu Pills and Gouqizi Dihuang Pills.
I was instructed to take the Huanglian Jiedu Pills every three days after meals and the Gouqizi Dihuang Pills before bedtime.
I would like to ask if it is safe to take Vitamin B and Choline simultaneously with Huanglian Jiedu Pills and Gouqizi Dihuang Pills.
Could this combination be harmful to my kidneys or detrimental to my health?

Dr. Yan Sumei reply Traditional Chinese Medicine

Hello! I am glad to receive your letter.
Regarding the question of whether taking Vitamin B, Xunli Ning, Huanglian Jiedu Pills, and Gouju Dihuang Pills together could harm the kidneys, it is important to first assess your kidney function.
If your kidney function is normal, you are young, and your metabolism is good, there is not much to worry about.
However, if your kidney function is impaired, it is not advisable to take these together.
When taking both traditional Chinese medicine and Western medicine, it is safer to space them out by at least one hour.
This is my response to you.
Wishing you good health and all the best! Sincerely, Yan Sumei, Department of Traditional Chinese Medicine, Nantou Hospital.

Combining Western medicine and Traditional Chinese Medicine (TCM) for kidney health can be a nuanced topic, especially considering the complexities of individual health conditions and the specific medications involved. In your case, you are taking Western medications (Cilostazol and Vitamin B) while also using TCM remedies (Huanglian Jiedu Pian and Guo Ju Di Huang Wan). Here’s a detailed exploration of the safety and considerations of this combination.

Safety of Combining Treatments
1. Understanding Each Medication:
- Cilostazol is primarily used to improve blood flow and is often prescribed for conditions like intermittent claudication. It can have side effects, including potential impacts on the liver and kidneys, especially in patients with pre-existing kidney conditions.

- Vitamin B supplements are generally safe and can be beneficial for overall health, including nerve function and energy metabolism.

- Huanglian Jiedu Pian is a TCM formula often used for detoxification and to clear heat, which may have diuretic effects.

- Guo Ju Di Huang Wan is typically used to nourish the kidneys and liver, and it may help with symptoms related to kidney deficiency.

2. Potential Interactions:
- While there is limited research on the specific interactions between these medications, the combination of Western and TCM treatments can sometimes lead to unexpected effects. For example, Huanglian Jiedu Pian may enhance the diuretic effect, which could potentially lead to dehydration or electrolyte imbalances, especially if you are also taking medications that affect kidney function.

- It is crucial to monitor how your body responds to this combination. Symptoms such as increased urination, changes in blood pressure, or any signs of kidney distress (like swelling, fatigue, or changes in urine output) should be reported to your healthcare provider immediately.

3. Kidney Health Considerations:
- Patients with kidney disease need to be particularly cautious with any medications, as the kidneys are responsible for filtering out drugs and their metabolites. The use of multiple medications can increase the burden on the kidneys.

- Regular monitoring of kidney function (e.g., serum creatinine, eGFR) is essential when combining treatments. This will help ensure that your kidneys are coping well with the medications.

Recommendations
1. Consult Your Healthcare Providers:
- It is vital to have open communication with both your Western medicine physician and your TCM practitioner. They should be aware of all the medications you are taking to avoid any potential interactions and to tailor your treatment plan accordingly.

- If possible, seek a healthcare provider who is knowledgeable in both Western and TCM practices, as they can provide a more integrated approach to your treatment.

2. Monitor Your Health:
- Keep track of any changes in your health status, including kidney function, and report these to your doctors. This includes any new symptoms or side effects you may experience after starting the TCM treatments.

- Regular blood tests and urine tests can help monitor your kidney function and ensure that the combination of treatments is not causing harm.

3. Lifestyle Considerations:
- In addition to medication, consider lifestyle factors that can support kidney health, such as maintaining a balanced diet, staying hydrated, managing blood pressure, and avoiding excessive protein intake, which can strain the kidneys.

Conclusion
In summary, while it is possible to combine Western and Traditional Chinese Medicine for kidney health, it must be approached with caution. The safety of this combination largely depends on individual health conditions, the specific medications involved, and ongoing monitoring of kidney function. Always consult with your healthcare providers before making any changes to your treatment regimen, and ensure that they are aware of all medications you are taking. This collaborative approach will help optimize your health outcomes while minimizing risks.
