Is it safe to take fish oil during the ninth month of pregnancy?

Hello, doctor.
I am currently 36 weeks pregnant and I take one to two fish oil capsules daily, which contain 180 mg of EPA and 120 mg of DHA.
Can I continue taking them until delivery, or should I stop? Will it affect coagulation function during labor? Thank you.

POPO, 30~39 year old female. Ask Date: 2014/04/15

Dr. Dong Yuhong reply Obstetrics and Gynecology

Hello: Fish oil is classified as a health supplement.
When selecting it, you should pay attention to whether it has the approval mark from the Ministry of Health and Welfare.
When taking it, be mindful of the labeled dosage and any related warnings.
If you are pregnant, it is advisable to inform your obstetrician-gynecologist.
Not all fish oil products available on the market are necessarily pure and natural.
Research has shown that long-term use may raise concerns regarding coagulation function, so if you are taking health supplements during pregnancy, it is recommended to communicate more with your physician.
Thank you!

More Info

Taking fish oil during pregnancy, especially in the later stages, is a topic that often raises questions among expectant mothers. Fish oil is rich in omega-3 fatty acids, particularly EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), which are known to have numerous health benefits for both the mother and the developing fetus. However, it is essential to consider the dosage and the timing of consumption.

In your case, you are currently 36 weeks pregnant and taking fish oil supplements that contain 180 mg of EPA and 120 mg of DHA. Generally, the consumption of fish oil during pregnancy is considered safe and can be beneficial. Omega-3 fatty acids are crucial for fetal brain and eye development, and they may also help reduce the risk of preterm birth and postpartum depression in mothers.

However, there are some important considerations to keep in mind:
1. Dosage: The American Pregnancy Association recommends that pregnant women consume at least 200 mg of DHA daily. Your current intake seems to meet this recommendation, but it is always best to consult with your healthcare provider regarding the appropriate dosage for your specific situation. Some studies suggest that higher doses may be beneficial, but this should be done under medical supervision.

2. Blood Coagulation: One of the concerns with taking fish oil, especially in higher doses, is its potential effect on blood coagulation. Omega-3 fatty acids can have a blood-thinning effect, which may increase the risk of bleeding during delivery. However, the typical doses found in over-the-counter fish oil supplements are usually not high enough to cause significant concerns. Nonetheless, it is crucial to discuss this with your healthcare provider, especially as you approach your due date.

3. Source of Fish Oil: Ensure that the fish oil you are consuming is of high quality and free from contaminants such as heavy metals and PCBs (polychlorinated biphenyls). Look for products that are certified for purity and safety.

4. Dietary Sources: In addition to supplements, consider incorporating dietary sources of omega-3 fatty acids into your meals. Fatty fish such as salmon, sardines, and mackerel are excellent sources. However, be cautious about the types of fish you consume, as some may contain higher levels of mercury, which can be harmful during pregnancy.

5. Consultation with Healthcare Provider: It is always advisable to consult with your obstetrician or healthcare provider before making any changes to your supplement regimen, especially in the final weeks of pregnancy. They can provide personalized advice based on your health history and any other medications or supplements you may be taking.

In summary, taking fish oil during the last month of pregnancy is generally safe and can provide significant benefits, but it is essential to monitor the dosage and consult with your healthcare provider to ensure it does not interfere with your delivery or coagulation status. Your healthcare provider can help you weigh the benefits against any potential risks and guide you on the best course of action as you approach labor.
