Itchy glans and painful foreskin during erection?

Hello Doctor, about two weeks ago, I started experiencing significant itching in my foreskin.
During showers, I noticed a lot of smegma (a visible white layer before washing), which I have never encountered before despite always maintaining good hygiene.
I'm worried that I might be ill.
Additionally, when rinsing, the itching feels similar to mosquito bites.
Although the amount of smegma has slightly decreased in the past few days, I've noticed that during erections, the foreskin feels tight and painful (it's almost impossible to clean the area under the glans when erect because pulling back the foreskin causes pain).
Moreover, contact with soap or body wash causes irritation.
All these symptoms have appeared recently, and I'm very concerned.
Could you please help me understand what this condition might be? Thank you, Doctor.

Hello, it is still necessary to enhance cleaning efforts.
For the time being, there may still be a sensation of pain.
It should improve after a while.
If the discomfort persists for more than two weeks, it is advisable to seek medical treatment.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Information obtained online should only be used as a reference; if necessary, it is important to visit a medical facility.

More Info

It sounds like you are experiencing a concerning set of symptoms related to your foreskin, including itching, an increase in smegma (the white substance you mentioned), and pain during erections. These symptoms can be indicative of several conditions, and it’s important to address them appropriately.

Possible Causes
1. Balanitis: This is an inflammation of the glans (the head of the penis) and can occur due to poor hygiene, infections (bacterial or fungal), or irritants such as soaps or lotions. The presence of smegma, which is a buildup of dead skin cells, oils, and other bodily fluids, can contribute to this condition.

2. Phimosis: This is a condition where the foreskin cannot be easily retracted over the glans. If you are experiencing tightness and pain during erections, it could be a sign of phimosis, which can lead to discomfort and hygiene issues.

3. Allergic Reaction or Irritation: The itching and pain you feel when using soap or body wash could indicate an allergic reaction or irritation to those products. It’s essential to use mild, unscented products to avoid exacerbating the issue.

4. Sexually Transmitted Infections (STIs): Certain STIs can cause symptoms like itching and pain. If you have had unprotected sexual contact, it may be wise to get tested.

5. Yeast Infection: A yeast infection can also cause itching and irritation, particularly if there is an overgrowth of Candida, a type of fungus.

Recommended Solutions
1. Improved Hygiene: Ensure that you are cleaning the area properly. Gently retract the foreskin (if possible) and wash with warm water. Avoid using harsh soaps or products that can irritate the skin.

2. Avoid Irritants: Switch to hypoallergenic and fragrance-free soaps and body washes. If you suspect a specific product is causing irritation, discontinue its use.

3. Topical Treatments: Over-the-counter antifungal creams may help if a yeast infection is suspected. If you suspect an allergic reaction, a mild hydrocortisone cream may reduce inflammation, but it’s best to consult a healthcare provider before applying any medication.

4. Consult a Healthcare Provider: Given the symptoms you are experiencing, it’s crucial to see a healthcare provider, preferably a urologist. They can perform a physical examination, provide a proper diagnosis, and recommend appropriate treatment. If phimosis is diagnosed, they may discuss options ranging from topical steroids to surgical interventions like circumcision.

5. Avoid Sexual Activity: Until you have seen a healthcare provider and received a diagnosis, it’s advisable to avoid sexual activity to prevent further irritation or potential transmission of infections.

Conclusion
Your symptoms are concerning and warrant professional evaluation. While some conditions can be managed with improved hygiene and over-the-counter treatments, others may require medical intervention. It’s essential to address these symptoms promptly to prevent complications and ensure your overall health and well-being. Please make an appointment with a healthcare provider as soon as possible to discuss your symptoms in detail and receive the appropriate care.
