Redness, swelling, and itching of the foreskin and glans?
Hello: I have a condition of phimosis, where the foreskin covers the penis both when erect and flaccid, and I can retract the foreskin (retracting it during an erection causes a feeling of swelling).
However, over the past year, I have frequently experienced symptoms of fissures in the foreskin, which has made normal sexual intercourse difficult.
Initially, the foreskin became red, swollen, and itchy (sometimes accompanied by white discharge).
Later, the tip of the foreskin began to fissure, and during sexual intercourse, the glans would swell, causing pain when retracting the foreskin! After sexual activity, my partner also experiences redness, swelling, itching, and excessive discharge in their intimate area.
I have consulted several hospitals about this situation! Some doctors said it was a yeast or fungal infection affecting the intimate area, while others suggested it was an infection caused by phimosis! They prescribed medications to apply regularly, but none have resolved the issue (it keeps recurring), which is very distressing! This condition continues to cause intermittent redness in my glans and foreskin; although sometimes it seems to self-heal if left alone, it can also become itchier at times! I really don't know how to permanently resolve this situation! I would like to consult a specialist: 1.
Is a circumcision necessary? 2.
If surgery is performed, will health insurance cover it? 3.
Are there any recommended hospitals or doctors in the northern region (Taipei, Taoyuan, Hsinchu, Miaoli) that can provide treatment?

Anonymous: Hello, 1.
Recurrent inflammation may necessitate circumcision.
2.
Whether health insurance covers the procedure depends on the specific medical condition.
Most circumcision surgeries are considered out-of-pocket expenses.
However, there are certain special circumstances outlined by health insurance that may cover related costs for circumcision.
It is advisable to consult with an outpatient physician.

Persistent redness and itching of the penis, particularly in the context of phimosis (a condition where the foreskin cannot be easily retracted), can be quite distressing and may indicate underlying issues that require careful evaluation and management. Based on your description, it seems you have been experiencing recurrent symptoms, including cracking of the foreskin, redness, swelling, and discharge, which have not responded well to various treatments. Here’s a comprehensive approach to your concerns:
1. Need for Circumcision: Given your history of recurrent infections and the symptoms associated with phimosis, a circumcision may be a viable option. Circumcision can alleviate the symptoms by removing the foreskin, which can harbor bacteria and fungi, leading to infections. It also allows for easier hygiene and reduces the risk of further irritation or injury to the glans (the head of the penis). However, the decision to proceed with surgery should be made after a thorough discussion with a urologist, who can assess the severity of your condition and the potential benefits of the procedure.
2. Insurance Coverage: In many regions, including the United States, health insurance may cover circumcision if it is deemed medically necessary, particularly if recurrent infections or phimosis are present. It is essential to check with your specific insurance provider and discuss with your healthcare provider to understand the coverage details and any necessary documentation required for approval.
3. Recommended Hospitals or Specialists: In the northern region of Taiwan (including Taipei, Taoyuan, Hsinchu, and Miaoli), there are several reputable hospitals and urologists who specialize in male genital health. It is advisable to seek a referral from your primary care physician or consult with a urologist who has experience in treating phimosis and related conditions. Look for hospitals with a strong urology department, such as National Taiwan University Hospital or Taipei Medical University Hospital, where you can receive comprehensive care.
4. Management of Current Symptoms: While considering surgical options, it is crucial to manage your current symptoms effectively. Here are some recommendations:
- Topical Treatments: Continue using antifungal or antibacterial creams as prescribed by your doctor. Ensure that you apply them consistently and as directed.
- Hygiene: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid harsh soaps or irritants that can exacerbate the condition.
- Avoid Irritants: Be mindful of any products that may irritate the area, including scented soaps, lotions, or condoms that may contain allergens.
- Moisturizing: Applying a gentle, fragrance-free moisturizer may help alleviate dryness and cracking of the foreskin.
- Avoid Sexual Activity: Until the symptoms are resolved, it may be wise to avoid sexual intercourse to prevent further irritation and potential transmission of infections to your partner.
5. Follow-Up Care: Regular follow-up with your healthcare provider is essential to monitor your condition and adjust treatment as necessary. If symptoms persist or worsen, further evaluation may be warranted to rule out other underlying conditions.
In conclusion, while circumcision may be a potential solution for your recurrent symptoms, it is essential to have a thorough discussion with a urologist to explore all options and ensure that you receive the most appropriate care for your situation. Your health and comfort are paramount, and addressing these concerns with a specialist will help you find relief and improve your quality of life.
