After a humeral fracture has healed, is it necessary to remove the metal pins?

After the car accident, I fractured my right humerus.
After being stabilized with six screws and one plate for over a year, I want to have the screws removed.
My attending physician said it may not be necessary to remove them because there are many nearby nerves, and there is a risk of nerve damage.
I have read many articles online, and most people have similar concerns.
So, should the screws be removed or not? It has been two years since my car accident.

mei, 20~29 year old female. Ask Date: 2014/05/04

Dr. Wang Shouji reply Orthopedics

Hello: Considering the risks associated with surgery, I also recommend that it may not be necessary to remove it, as scarring in the surrounding soft tissue after surgery can increase the likelihood of nerve injury.
If nerve damage occurs, the potential for long-term complications is significant, so I suggest that removal may not be necessary.
Thank you!

When it comes to the decision of whether to remove steel pins after a humerus fracture has healed, there are several factors to consider. The primary concern is whether the presence of the pins is causing any functional issues or discomfort. In many cases, if the pins are not causing any problems, they can remain in place without any adverse effects.

Risks of Leaving Steel Pins in Place
1. Infection: While the risk of infection is generally low, any foreign body can potentially become a site for infection. If the pins are not causing any symptoms, the risk is minimal.

2. Nerve Damage: As you mentioned, there are many nerves in the vicinity of the humerus. Removing the pins can pose a risk of damaging these nerves, which could lead to complications such as loss of function, pain, or numbness.

3. Chronic Pain or Discomfort: Some patients report chronic pain or discomfort associated with the presence of metal hardware. If you are experiencing such symptoms, it may be worth discussing with your doctor the possibility of removal.

4. Metal Sensitivity: Although rare, some individuals may develop sensitivity or allergic reactions to the metal used in the pins. If you suspect this might be the case, it’s important to consult with your healthcare provider.

Benefits of Removing Steel Pins
1. Elimination of Foreign Body: Removing the pins eliminates the foreign body from your body, which can sometimes alleviate discomfort or pain.

2. Improved Range of Motion: Some patients report improved range of motion and less stiffness after the removal of hardware, although this is not universally experienced.

3. Peace of Mind: For some, the psychological benefit of knowing that the hardware is no longer present can be significant.

Factors to Consider
1. Time Since Injury: Since your injury occurred two years ago, the bone should be well-healed. However, the decision to remove the pins should be based on your current symptoms and the advice of your orthopedic surgeon.

2. Current Symptoms: If you are experiencing pain, discomfort, or functional limitations, it may be worth discussing the removal of the pins with your doctor. If you are not experiencing any issues, it may be advisable to leave them in place.

3. Consultation with a Specialist: It may be beneficial to seek a second opinion from another orthopedic specialist. They may provide additional insights or alternative recommendations based on their experience.

4. Imaging Studies: If you haven’t had recent imaging studies (like X-rays) to assess the condition of the pins and surrounding bone, it may be a good idea to do so. This can help determine if there are any complications that need to be addressed.

Conclusion
In summary, the decision to remove steel pins after humerus fracture healing should be made based on a thorough evaluation of your current symptoms, the risks associated with removal, and the potential benefits. If the pins are not causing any issues, many surgeons recommend leaving them in place. However, if you are experiencing discomfort or have concerns about the hardware, it is important to have an open discussion with your orthopedic surgeon about your options. They can provide personalized advice based on your specific situation and medical history.
