Palpitations and chest tightness?

Hello doctor, I have mild mitral and tricuspid valve prolapse, but I am not on any medication.
Recently, I have been experiencing frequent palpitations, and my heart seems to skip a beat, especially noticeable when I just wake up.
This has happened many times over the past few weeks.
I had an electrocardiogram that came back normal, and the physician explained that it was early heart contractions.
They prescribed Inderal for me to take when I feel uncomfortable, but I thought it would be fine not to take the medication.
However, I have noticed some tightness in my chest recently, and my breathing feels strange.
When I take a deep breath, it feels like something is pressing on my chest.
It’s not painful, but the muscles in the upper left part of my chest suddenly tighten.
I have taken the medication for a few days, but I still feel this strange sensation in my chest.
Doctor, could this be a heart issue or something else?

Hello, have you been under a lot of stress lately? Or have you been having trouble sleeping? Mitral valve prolapse could potentially cause the symptoms mentioned, but they are more likely related to autonomic nervous system dysfunction.
If taking Inderal is not effective, it may be advisable to consult a physician at the outpatient clinic for medication adjustment.

Understanding heart palpitations and chest discomfort can be quite concerning, especially when you have a pre-existing condition like mild mitral and tricuspid valve prolapse. It’s important to recognize that while these symptoms can be related to heart issues, they can also stem from other causes, including anxiety, stress, or even gastrointestinal problems.

Heart Palpitations
Palpitations are sensations that feel like your heart is pounding, fluttering, or beating irregularly. They can occur in healthy individuals and are often benign. In your case, the palpitations you experience, particularly upon waking, may be attributed to several factors:
1. Mitral Valve Prolapse (MVP): MVP is a common condition where the mitral valve doesn't close properly, which can lead to irregular heartbeats or palpitations. While many people with MVP do not experience significant symptoms, some may feel palpitations or discomfort.

2. Early Heartbeats: Your doctor mentioned "premature contractions," which are common and usually harmless. They occur when the heart's electrical system misfires, causing an early heartbeat. These can be exacerbated by stress, caffeine, or fatigue.

3. Anxiety and Stress: Emotional factors can significantly influence heart rhythm. Anxiety can lead to increased awareness of heartbeats and may cause physical symptoms like tightness in the chest or difficulty breathing.

Chest Discomfort
The sensation of chest tightness or discomfort, especially if it feels like pressure or heaviness, can be alarming. Here are some considerations:
1. Musculoskeletal Issues: Sometimes, chest discomfort can arise from muscle strain or tension, particularly if you have been under stress or have changed your physical activity levels. This can lead to tightness in the chest muscles, which may feel like pressure.

2. Gastroesophageal Reflux Disease (GERD): Acid reflux can cause a sensation of tightness or discomfort in the chest, often mistaken for heart-related issues. This is especially relevant if you notice that your symptoms worsen after eating or when lying down.

3. Cardiac Concerns: While your ECG was normal, persistent symptoms warrant further evaluation. Conditions like ischemia or other heart diseases can present with similar symptoms. Given your history of valve prolapse, it’s wise to monitor these symptoms closely.

Recommendations
1. Medication: You mentioned being prescribed Inderal (propranolol), which is a beta-blocker that can help manage palpitations and anxiety. If you find that your symptoms are frequent and bothersome, it may be beneficial to take the medication as prescribed.

2. Lifestyle Modifications: Consider lifestyle changes that can help reduce palpitations and chest discomfort:
- Reduce Caffeine and Alcohol: Both can exacerbate palpitations.

- Manage Stress: Techniques such as yoga, meditation, or deep-breathing exercises can help.

- Regular Exercise: Engaging in regular physical activity can improve overall cardiovascular health and reduce anxiety.

3. Follow-Up: Since you are experiencing new or worsening symptoms, it’s crucial to follow up with your healthcare provider. They may recommend further testing, such as a Holter monitor to assess your heart rhythm over a longer period or an echocardiogram to evaluate your heart's structure and function.

4. Emergency Symptoms: Be vigilant for any signs of more serious conditions, such as severe chest pain, shortness of breath, or symptoms that radiate to the arm, neck, or jaw. If you experience these, seek immediate medical attention.

Conclusion
While your symptoms may be related to your existing mitral valve prolapse and could be benign, it’s essential to remain proactive about your health. Regular check-ups and open communication with your healthcare provider will help ensure that any potential issues are addressed promptly. Remember, it’s always better to err on the side of caution when it comes to heart health.
