Regarding the injury to the glans sulcus after erection?

Hello, Doctor.
I am 37 years old and have been experiencing symptoms for the past three months.
Whenever I have an erection that is too firm, I get fissures in the glans sulcus.
I had a circumcision around the age of 10, and everything has been normal since then.
I have two children.
I have previously experienced hard erections that caused fissures, but recently, it seems that any erection leads to these fissures, including during natural morning erections.
This has been very distressing for me, especially during intercourse, as it feels uncomfortable.
It seems like the corpora cavernosa are expanding too much and causing the glans sulcus to split open.
Could it be that the circumcision I had at age 10 left the remaining foreskin too short? I'm quite troubled by this and would appreciate any advice you may have.

Using adequate lubrication before intercourse can reduce the risk of tearing.
If it continues to occur repeatedly, it is advisable to consult a physician for a thorough evaluation.

More Info

Understanding glans fissures after erection can be a concerning issue, especially when it leads to discomfort during sexual activity. Based on your description, it seems that you are experiencing recurrent fissures or cracks in the glans (the head of the penis) after achieving an erection, which has been increasingly problematic over the past three months.

Causes of Glans Fissures
1. Tight or Short Frenulum: After circumcision, if the remaining skin is too tight or if the frenulum (the band of tissue on the underside of the penis) is short, it can lead to excessive tension during erections, resulting in fissures.

2. Skin Conditions: Conditions such as eczema, psoriasis, or lichen sclerosus can cause dryness and cracking of the skin. If you have a history of skin issues, this could be a contributing factor.

3. Inadequate Lubrication: During sexual activity, insufficient lubrication can lead to friction, which may cause the skin to crack. This is particularly relevant if you are experiencing discomfort during intercourse.

4. Increased Rigidity of Erection: If your erections are particularly hard, it can put additional stress on the skin of the glans, leading to fissures. This could be due to various factors, including hormonal changes or psychological factors.

5. Infection or Inflammation: Conditions such as balanitis (inflammation of the glans) can also lead to fissures. This may be accompanied by redness, swelling, or discharge.

Solutions and Recommendations
1. Consult a Urologist: The first step is to consult a urologist who can perform a thorough examination. They can assess whether the issue is related to the tightness of the remaining foreskin or frenulum, or if there are other underlying conditions.

2. Moisturizing Creams: Using a gentle, fragrance-free moisturizer or emollient on the glans can help keep the skin hydrated and reduce the likelihood of cracking. Avoid products with alcohol or strong fragrances, as these can irritate the skin.

3. Lubrication During Intercourse: If you are experiencing discomfort during sexual activity, using a water-based or silicone-based lubricant can help reduce friction and prevent fissures.

4. Gentle Handling: Be mindful during erections and sexual activity. Avoid excessive force or rough handling, which can exacerbate the issue.

5. Consider Frenuloplasty: If the frenulum is determined to be too short or tight, a minor surgical procedure called frenuloplasty may be recommended. This procedure can relieve tension and prevent further fissures.

6. Monitor for Infection: If you notice any signs of infection, such as increased redness, swelling, or discharge, seek medical attention promptly.
7. Avoid Irritants: Be cautious with soaps and hygiene products that may irritate the genital area. Opt for mild, hypoallergenic products.

Conclusion
It is understandable that this situation is causing you distress, especially given its impact on your sexual health and comfort. The key is to seek professional medical advice to determine the underlying cause of your symptoms and to explore appropriate treatment options. With the right approach, you can manage this condition effectively and improve your quality of life.
